糖尿病患者白内障超声乳化手术后黄斑改变的临床观察

摘　　要：目的探讨糖尿病患者白内障超声乳化手术后黄斑区结构的变化情况。方法选取2011年1月-2012年12月在本院行超声乳化白内障摘除术联合人工晶状体植入术的患者70例70眼的资料进行回顾性分析,其中糖尿病组患者35例35眼,对照组患者35例35眼,超声乳化白内障摘除术后观察6个月,对比术前及术后1个月两组患者的术眼黄斑区厚度、视力。结果对照组术前及术后1个月的黄斑中心凹厚度分别为（146.3±2.5）μm和（147.6±3.0）μm,差异无统计学意义（t=1.883,P=0.064）;糖尿病组术前及术后1个月的黄斑中心凹厚度分别为（147.7±1.8）μm和（199.7±18.2）μm,差异有统计学意义（t=16.776,P〈0.01）;术后1个月两组的黄斑中心凹厚度比较,差异有统计学意义（t=16.679,P〈0.01）。术后1个月糖尿病组与对照组的视力分别为0.34±0.25、0.13±0.12,差异有统计学意义（t=4.474,P〈0.01）。结论糖尿病患者在白内障超声乳化摘除术后黄斑中心凹厚度明显增加,黄斑水肿发生率高。Objective To investigate the changes of macula after phacoemulsification in diabetics. Methods A retro- spective analysis was made on 70 patients who underwent phaeoemulsifieation in our hospital from January 2011 to December 2012.35 cases of 70 patients with diabetes were in diabetic group,and others were in control group.Postopera- tive follow-up lasted for 6 months after phacoemulsification.Contrasts were made on the thickness of the fovea of retina before phacoemulsification and 1 month after surgery. Results In control group,the mean fovea thickness was （146.3± 2.5） μm preoperatively and （147.6±3.0） μm 1 month postoperatively,the difference was not significant （t=1.883,P= 0.064）.In diabetic group,the mean fovea thickness was （147.7±1.8）μm preoperatively and （199.7±18.2） μm 1 month postoperatively,the difference was significant （t=16.776,P〈0.01）.Thc difference of the mean fovea thickness between the two groups 1 month after surgery was significant （t=16.679,P〈0.01）.The vision of diabetic group and control group 1 month after surgery was 0.34-＋0.25 and O.13-＋0.12,the difference was significant （t=4.474,P〈0.0l）. Conclusion The thickness of the fovea of retina increases significantly and the morbidity of macula edema increases as well after pha- coemulsification in diabetics.
