摘 要:目的探讨细胞周期素D1(Cyclin D1)和多肿瘤抑制基因1(P16)的表达与下咽鳞状细胞癌(简称"鳞癌")临床病理特征和预后的关系。方法免疫组织化学方法检测50例下咽鳞癌和20例正常下咽组织中Cyclin D1和P16的表达,并结合下咽癌的病理学行为和临床随访资料进行分析。结果下咽鳞癌中Cyclin D1表达阳性率(72.0%)明显高于正常下咽组织(25.0%)(P<0.05),而P16表达阳性率(42.0%)明显低于正常下咽组织(90.0%)(P<0.05)。Cyclin D1过表达和P16表达缺失与下咽癌的病理分化程度、T分期、颈淋巴结转移、患者生存期均有关(均P<0.05)。Cyclin D1与P16的表达呈负相关(r=-0.68,P<0.05)。结论 Cyclin D1过表达与P16表达缺失可能参与调节下咽鳞癌的发生发展,Cyclin D1和P16存在互相抑制倾向,Cyclin D1和P16表达水平可以作为评估下咽鳞癌生物学行为和预后的参考指标。Objective To investigate the expression of Cyclin D1 and multiple tumor suppressor gene 1(P16) in hypopharyngeal squamous cell carcinoma and their relationship with pathological behavior and prognosis. Methods Immunohistochemical technology was applied to detect the expression of Cyclin D1 and P16 in 50 cases of hypopharyngeal squamous cell carcinoma and 20 cases of normal hypopharyngeal tissues. The study was also combined with analysis of the pathological behavior and follow-up data. Results In hypopharyngeal squamous cell carcinoma, the positive rate of Cyclin D1(72.0%) was higher than normal hypopharyngeal tissues(25.0%)(P〈0.05), but positive rate of P16(42.0%)was lower than that of normal hypopharyngeal tissues(90.0%)(P〈0.05). The over expression of Cyclin D1 and low expression of P16 were significantly related with pathological differentiation, T staging, cervical lymph node metastasis of hypopharyngeal squamous cell carcinoma and patients survival(P〈0.05). The expression of Cyclin D1 and P16 was correlated negatively(r =-0.68, P〈0.05). Conclusion The over expression of Cyclin D1 and the low expression of P16 may be involved in regulating the occurrence and development of hypopharyngeal squamous cell cancer. The expression of Cyclin D1 is negatively correlated with P16. The expression levels of Cyclin D1 and P16 can be used as the reference index to evaluate the biological behavior and prognosis of hypopharyngeal squamous cell carcinoma.
