超声结合血清中CA125对绝经后卵巢恶性肿瘤的诊断价值

摘　　要：目的探讨超声结合血清中CA125对绝经后卵巢恶性肿瘤的诊断价值。方法回顾性分析2012年4月-2014年4月鞍山市肿瘤医院妇产科收治的100例绝经后卵巢肿瘤患者的临床资料,按照检查方法分为超声组（30例）、CA125组（30例）、超声联合CA125组（40例）。所有患者均给予子宫全切除手术或者次全切除手术,且术中进行冰冻切片病理检查,根据病理检查结果观察血清CA125检测的特异性、敏感性。结果超声联合CA125组的确诊率为32.5%,显著高于超声组、CA125组（10%、12%）,差异均有统计学意义（P〈0.05）。血清中CA125的值以35 U/mL为诊断界点时,随着值的增大,卵巢恶性肿瘤发生率显著增高（P〈0.05）;CA125的诊断界点从35 U/mL提高到80 U/mL时,诊断特异性从94.8%提高至98.9%,但诊断敏感性从55.6%降低到49.5%;超声下显示为实性的肿瘤,诊断为恶性肿瘤的概率显著增大（P〈0.05）。结论超声诊断结合血清CA125测定对于鉴别诊断绝经后卵巢良恶性肿瘤价值重大。Objective To discuss the value of ultrasound combined with serum CA125 in the diagnosis postmenopausal malignant neoplasm of ovary. Methods Clinical data of 100 patients with postmenopausal neoplasm of ovary in Tumor Hospital of Anshan City from April 2012 to April 2014 were retrospectivey analyzed. All the patients were divided into the ultrasound group（30 cases）, CA125 group（30 cases）, and ultrasonic combined CA125 group（40 cases） according to the inspection methods, all patients were given total excision surgery or subtotal resection surgery of the uterine, and frozen section pathological examination was done during operation. According to the pathologic examination results, the specificity and sensitivity of serum CA125 detection were observed. Results The positive rate of the ultrasonic combined CA125 group was 32.5%, which was significantly higher than the ultrasound group and CA125 group（10%, 12%）, the differences were statistically significant（P〈0.05）. 35 U/mL was conducted as the diagnostic point of CA125, with the increased of level of serum CA125, the incidence of ovarian malignant tumors was significantly increased（P〈0.05）;when the diagnostic point of the CA125 increased from 35 U/mL to 80 U/mL, the diagnostic specificity increased from94.8% to 98.9%, but the diagnostic sensitivity reduced from 55.6% to 49.5%. If the tumor was displayed for solid tumor under the ultrasound, the probability of diagnosis of malignant tumors has significant increased（P〈0.05）. Conclusion The diagnosis value of ultrasound combined with serum CA125 in postmenopausal malignant neoplasm of ovary is very great.
