摘　　要：目的探讨单孔腹腔镜手术在治疗小儿子宫圆韧带囊肿中的应用。方法2008年1月至2014年8月共收治d,JL子宫圆韧带囊肿33例，年龄1．5—6．5岁，平均3．6岁。所有病例均为单侧，其中左侧8例，右侧25例，术前均予彩超检查进行确诊，所有病例选择单孔腹腔镜手术进行鞘状突高位结扎。结果33例手术均在腹腔镜下顺利完成，手术时间为5—20．5min，平均10．4min，术中出血量为0．5～5ml，平均1．2ml。患儿麻醉完全清醒后（平均时间为术后2h）给予全流或半流饮食，并可下床活动，未发现有脐戳孔疝、戳口处血肿、腹膜外气肿等并发症，患儿均在术后第2天出院。术后1个月、6个月、24个月、48个月、5年随访并复查彩色超声。30例获得随访0．5—5年，平均3．4年，无并发症及复发病例。结论单孔腹腔镜鞘状突高位结扎术治疗小儿子宫圆韧带囊肿初步结果显示安全有效，操作简便，而且具有创伤小，术后恢复快、不留手术瘢痕等优点，值得临床进一步研究。Objective To discuss the application of Single-port laparoscopic herniorrhaphy for pediatric uterine round ligament cyst. Methods From Jan. 2008 to Aug. 2014, 33 children with uterine round ligament cyst were enrolled in this study. The patients＇ age range from 1.5 to 6.5 years, mean age was 3.6 years. All cases were unilateral, including 8 cases on the left side and 25 cases on the right side. All patients were confirmed by ultrasound examination preoperation and undergone high ligation surgery with a single-port laparoscopic technique. Results All surgeries were successful. Operation time was 5 to 20.5 rain, mean time was 10.4 rain. Bleeding volume during the operation was 0.5 to 5 ml, mean volume was 1.2 ml. The patients could eat and exercise as usual after operation in that day. Complications such as trocar site hernia or hematoma and extraperitoneal emphysema was not detected. After 2 days of operation, the patients were discharged, All children were followed- up 1,6, 24, 48 months and five years after surgery and reviewed of color doppler uhrasonography. And 30 patients were successfully followed-up 6 months to five years, mean visit time was 25.4 months. There were no complication and recurrence in this study. Conclusion Single-port laparoscopic herniorrhaphy for pediatric uterine round ligament cyst was safe, effective, and micro-invasive, which is worth of being generalized.
