23例脑部多形性黄色星形细胞瘤的MRI分型对比研究

摘　　要：目的探讨脑部多形性黄色星形细胞瘤(PXA)不同影像学类型间的临床、MRI及病理特点。方法回顾性分析经手术病理证实的23例脑部PXA(囊性型12例,实性型11例)患者术前MRI、临床及病理资料,比较两型病变在发病年龄、瘤体大小、瘤周水肿程度、发病部位、恶性率等方面的差别。结果囊性型PXA患者的发病年龄平均(19±7.2)岁,实性型患者的发病年龄平均为(32±9.7)岁,两者发病年龄差异有统计学意义(P=0.002);囊性型PXA病变体积中位数为113902.1 mm3,明显大于实性型(45731.75 mm3,P=0.000)。两型病变在发病部位、瘤周水肿程度方面差异无统计学意义(P=0.427,P=1.000)。病理上,囊性型恶性率为25%(3/12),实性型恶性率为9%(1/11)。结论 PXA在不同影像学类型的表现有一定程度的差异,这或许有助于该病的分层治疗。Objective To investigate the clinical, MRI and pathological characteristics of different subtypes of cerebral pelomorphic xanthoastrocytoma （PXA）. Methods Twenty-three patients with pathologically-proved PXA were enrolled in this study. The PXA lesions included cystic type （n = 12） and solid type （n = 11 ）. The preoperative MRI features, the clinical manifestations and the pathological findings were retrospectively analyzed. The results were compared between the two subtypes, focusing on the patient＇ s age, tumor size, peritumoral edema, tumor location, malignancy ratio, etc. Re- stilts The mean age of onset of cystic PXA and solid PXA was （ 19 ± 7.2） years old and （ 32 ± 9.7 ） years old respective- ly, and the difference between the two types was statistically significant （ P = 0. 002 ）. The median volume of cystic PXA lesion was 113902.1 mm3 , which was significantly larger than that of solid PXA lesion （45731.75 mm3 , P =0. 000）. No statistically significant differences in the tumor size and the peritumoral edema existed between the two types. Pathological- ly, the malignancy ratio of cystic and solid PXA was 25% （3/12） and 9% （ 1/11 ） respectively. Conclusion Different pathological type of PXA has different imaging features, which might be helpful for the stratification treatment of PXA.
