血液C-反应蛋白水平与抑郁症关系的系统评价

摘　　要：目的：探讨社区和临床人群中血液C-反应蛋白（C-reactive protein,CRP）水平与抑郁症的关系。方法：使用Cochrane Library、Pub Med、Web of knowledge、Ebsco、中国生物医学文献数据库、中国学术期刊全文数据库、万方数据库以及维普中文科技期刊全文数据库,检索和筛选抑郁症CRP水平的病例对照研究,检索日期至2013年10月。对纳入文献进行质量评价和数据提取后,用Review manager 5.0软件进行Meta分析。结果：Meta分析纳入文献共13篇,其中抑郁患者2 779例,正常对照人群6 189例。结果显示男性、女性及总抑郁人群血液CRP均高于正常对照人群,结果具有统计学差异：总人群WMD=0.32（95%CI=0.13~0.51,P=0.000）;男性人群WMD=0.33（95%CI=-0.02~0.69,P=0.070）;女性人群WMD=0.29（95%CI=0.06~0.52,P=0.020）。结论：抑郁症患者血液CRP水平明显增高,对抑郁症的早期发现和准确诊断具有重要的指导意义。Objective：To assess the association of depression with C-reactive protein（CRP）in community and clinical samples.Methods：Databases of Cochrane Library,Pubmed,Web of Knowledge,Ebsco,CBM,CNKI,CECDB and CQVIP were used to search and filter relevant literatures of CRP level of depression up to October 2013. The quality of the literatures was assessed and the data were extracted. Review manager 5.0 was used to do analysis. Results：Thirteen literatures were enrolled in the study including 2 779 depression patients and 6 189 healthy controls. The results showed that CRP were higher in the male,female and the total population than in the control,with statistical differences：total WMD=0.32（95%CI=0.13 to 0.51,P=0.000）;male WMD=0.33（95%CI=-0.02 to 0.69,P=0.070）;women WMD=0.29（95%CI=0.06 to 0.52,P=0.020）. Conclusion：Depression and CRP are positively associated in the clinical and community samples,which is useful for the early detection of depression and the accurate diagnosis of depression.
