健脾益气法联合化疗治疗晚期大肠癌临床观察

摘　　要：目的:观察扶正健脾方药联合化疗治疗晚期大肠癌的临床疗效。方法:将本院肿瘤科2011年2月—2013年12月共74例大肠癌晚期患者,随机分为联合治疗组37例,单纯化疗组37例。联合治疗组采用FOLFOX方案化疗联合扶正健脾方药治疗,单纯化疗组应用FOLFOX方案化疗。观察两组患者治疗后近期实体瘤疗效、脾虚证候积分、生存质量(KPS评分)及免疫功能变化情况。结果:联合治疗组、单纯化疗组总有效率分别为16.2%和8.10%(P>0.05);疾病控制率分别为83.8%和48.6%(P<0.01)。联合治疗组治疗后脾虚证候积分明显下降(P<0.05),单纯化疗组则无明显改善(P>0.05);组间治疗后比较,差异有统计学意义(P<0.05)。联合治疗组治疗后KPS评分升高(P<0.05),单纯化疗组则无明显改善(P>0.05);组间治疗后比较,差异有统计学意义(P<0.05)。联合治疗组治疗后免疫细胞NK及CD3水平明显升高(P<0.05),CD8水平下降(P<0.05);单纯化疗组则无明显变化(P>0.05);组间治疗后比较,CD3、CD4、CD8、NK差异有统计学意义(P<0.05)。结论:扶正健脾方药联合化疗有明确稳定肿瘤、改善脾虚状况、提高患者生存质量和改善免疫功能的作用。Objective： To evaluate the clinical efficacy of spleen-nourishing and qi-boosting method combined with chemotherapy in patients with advanced colorectal cancer. Methods： From February 2011 to December 2013,we collected 74 cases of patients with advanced colorectal cancer who were from our hospital in total and put them into two groups randomly. 37 cases were in the combined treatment group who used FOLFOX and TCM and the other 37 cases were in the chemotherapy group who used FOLFOX. After the treatment,we evaluated the effect,spleen symptoms integration,quality of life（KPS score） and changes in immune function. Results： The effective rate was 16. 2% in the combined treatment group and 8. 10% in the chemotherapy group respectively（P〉0. 05）. The disease control rate was 83. 8% and 48. 6% respectively（P〈0. 01）. The symptom scores of spleen deficiency decreased and the score of KPS increased in the combined treatment group（P〈0. 05） while there was no difference in the chemotherapy group（P〉0. 05）. And the level of CD3 and NK increased while the level of CD8 decreased only in the combined treatment group（P〈0. 05）. There was no difference between groups（P〈0. 05）. Conclusion： The spleen-nourishing and qi-boosting method combined with chemotherapy can control the patients＇ tumor size,improve Spleen deficiency,the quality of life and immune function.
