摘  要:目的:探讨血清NSE、CRP、SCCA、Cyfra21-1及胸腔积液中NSE、Cyfra21-1在肺癌的病理分型、临床分期及联合诊断的应用价值。方法:应用化学发光法检测100例肺癌患者和50例感染性肺部疾病患者血清中NSE、Cyfra21-1、SCCA、CRP的浓度及胸水中NSE、Cyfra21-1的浓度。结果:肺癌血清中SCCA、Cyfra21-1、NSE及胸水中NSE、Cyfra21-1的阳性率均显著高于感染性疾病组(P<0.05)。血清中SCCA、Cyfra21-1、NSE阳性率在Ⅲ、Ⅳ期均显著高于Ⅰ、Ⅱ期,血清中CRP和胸水中Cyfra21-1、NSE阳性率在Ⅱ-Ⅳ期均显著高于Ⅰ期(P<0.05)。鳞癌中血清和胸水Cyfra21-1阳性率最高(P<0.05),小细胞肺癌中血清和胸水NSE阳性率最高(P<0.05),鳞癌中血清SCCA、CRP阳性率最高(P<0.05)。联合检测血清四种标志物与单项检测相比,其灵敏度升高,特异性降低(P<0.05),而联合检测与单项检测胸水Cyfra21-1、NSE相比灵敏度和特异性无明显变化(P>0.05)。结论:血清NSE、Cyfra21-1、CRP、SCCA及胸腔积液中NSE、Cyfra21-1在肺癌的诊断和鉴别诊断中起着重要的作用,并且有助于临床鉴别肺癌的分型和分期,联合检测有利于肺癌的早期诊断。Objective:To investigate the applied value of serum Cyfra21-1, NSE, CRP and SCCA and pleural effusion Cyfra21- 1, NSE in pathological types, clinical stages and combination detection for diagnosis of lung cancer. Methods: Based on chemiluminescence immunoassay,the levels of serum NSE ,Cyfra21-1 ,SCCA,CRP and pleural effusion NSE,Cyfra21-1 in 100 patients with lung cancer (case group) and 50 patients with benign diseases (control group) were determined. Results: The positive rates of serum Cyfra21-1 ,NSE,SCCA and pleural effusion NSE,Cyfra21-1 were significantly higher in ease group than those in control group (P〈O. 05 ). The positive rates of serum SCCA, Cyfra21-1, NSE in stage III, IV were higher than stage I , H , the positive rates of serum CRP and the pleural effusion Cyfra21-1 ,NSE in stage II-IV were higher than stage I (P〈0. 05 ). The positive rates of serum and pleural effusion Cyfra21-1 were highest in squamous carcinoma ( P〈O. 05 ), the positive rate of serum and pleural effusion NSE was highest in small cell carcinoma ( P 〈 0.05 ), and the positive rate of serum SCCA and CRP in patients were highest in squamous carcinoma (P〈0. 05 ). The sensitivity of the combination detection of serum tumor markers in diagnosing lung cancer was significantly enhanced (P〈0.05),however,the sensitivity of the combination detection of pleural effusion Cyfra21-1, NSE were not enhanced( P〉 0.05). Conclusion:The serum NSE, Cyfra21-1, CRP, SCCA and pleural effusion NSE, Cyfra21-1 play an important role in the diagnosis of lung cancer and contribute to the pathological type and TNM stage of lung cancer, the combination detection of tumor markers is helpful for the early diagnosis of lung cancer.
