摘 要:目的 探讨长期、严重、广泛虹膜后粘连情况下瞳孔闭锁、瞳孔膜闭的瞳孔成形术.方法 采用高频电子撕囊仪对瞳孔缘或部分虹膜实施环形切割,并与传统的瞳孔缘放射状切开及环形剪除进行对比.结果 高频切割组的瞳孔可散大至6 mm以上,充分止血,无虹膜漂移现象,术后虹膜的炎性反应,瞳孔再粘连的发生率明显低于传统法.结论 对于严重虹膜后粘连,采用高频切割的方法能够顺利地完成白内障及玻璃体切除手术,并发症显著降低.Objective To study coreoplasty for pupil seclusion and pupil occlusion under the condition of long-term,serious and extensive posterior synechia.Methods The high frequency electronic capsulorhexis instrument was used to perform a circular cutting on pupillary margin or adjacent iris.The operative effect was compared with that of traditional pupillary margin radial iridotomy and circular cutting.Resuits The pupillary diameters of high frequency cutting group were more than 6mm and there were no phenomenon of iris drift.Compared with traditional method,it had lower incidences of inflammatory response and pupil adhesion.Conclusion The high frequency cutting method has lower complications and could improve success rate of cataract and vitrectomy surgery for patients with serious posterior synechia.
