卵巢癌患者血清和肿瘤组织HE4和CA125联合检测的临床意义

摘　　要：目的：探讨联合检测血清和肿瘤组织HE4和CA125水平对卵巢癌患者的临床意义。方法检测术前及术后卵巢癌组（ A组）、卵巢良性肿瘤组（ B组）及其他卵巢疾病组（ C组）患者血清HE4和CA125水平，及卵巢组织中HE4和CA125含量。1年后测定卵巢癌患者的血清HE4和CA125水平。综合分析各统计指标的相关性。结果 A组癌组织HE4和CA125阳性表达率均明显高于B组瘤组织和C组组织。 A组术前血清HE4和CA125水平明显高于B组和C组（P＜0．01）；B组术前CA125水平高于C组（P＜0．01）。术前Ⅲ～Ⅳ期卵巢癌患者血清HE4和CA125水平明显高于Ⅰ～Ⅱ期（P＜0．01），术后HE4和CA125水平均低于术前（P＜0．01）。出院后1年卵巢癌组复发患者血清HE4和CA125水平明显高于稳定患者（P＜0．05）。血清HE4联合CA125检测对卵巢癌诊断的敏感性和特异性均高于任何一项单独检测。结论联合检测组织和血清中的HE4和CA125可提高卵巢癌诊断的敏感性和特异性，对卵巢癌的早期诊断、疗效评估和病情监测具有重要价值。Objective To investigate the clinical significance of combined detection of HE 4 and CA125 in the serum and tumor tissues of patients with ovarian cancer .Methods Preoperative and postoperative serum level and content of HE 4 and CA125 in the ovarian cancer group（Group A,75 cases）,the benign ovarian tumor group（Group B,65 cases）and the non ovarian group（Group C,45 cases）were detected.Serum level of HE4 and CA125 of the ovarian cancer patients was measured in 1 year, correlation of statistical indexes was comprehensively analyzed .Results The positive expression rates of HE 4 and CA125 in the Group A were much higher than those of Group B and C;The preoperative serum level of HE 4 and CA125 in the Group A was sig-nificantly higher than that of Group B and C group （P〈0.01）;The preoperative CA125 level in Group B were higher than that of Group C（P〈0.01）.The preoperative serum level of HE4 and CA125 of stage Ⅲ~Ⅳ ovarian cancer was significantly higher than that stageⅠ-Ⅱovarian cancer（P〈0.01）,the postoperative HE4 and CA125 level was lower than that before operation （P〈0.01）.Serum level of HE4 and CA125 of discharged patients with recurrent ovarian cancer in the Group A after 1 year were significantly higher than that of the stable patients （P〈0.05）.The sensitivity and specificity of combined detection of HE 4 and CA125 in the diagnosis of ovarian cancer were higher than any single detection .Conclusion The combined detection of HE 4 and CA125 in tissues and serum can improve the sensitivity and specificity in the diagnosis of ovarian cancer ,and it has important val-ue for early diagnosis ,curative effect evaluation and illness monitoring of ovarian cancer .
