支气管封堵器在胸腹腔镜联合食管癌根治术中的应用

摘　　要：目的探讨支气管封堵器在胸腹腔镜联合食管癌根治术中的应用价值。方法对50例食管癌患者采用单肺通气麻醉下行胸腹腔镜联合食管癌根治术,按照麻醉方式的不同,分为A组(n=25例)和B组(n=25例)。A组采用支气管阻塞器技术,B组采用双腔气管插管技术。分析2种通气方式在胸腹腔镜联合食管癌根治术中对患者单肺通气的影响。结果单肺通气30 min后,A组和B组动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、动脉血氧饱和度(SaO2)及气道峰压(Paw)等指标进行比较,差异无统计学意义(P>0.05);A组和B组的插管定位时间、气道峰压增加率、咽痛声嘶发生率等指标进行比较,差异有统计学意义(P<0.05);术野暴露及肺塌陷情况满意率等指标进行比较,差异无统计学意义(P>0.05)。结论采用支气管封堵器行单肺通气在插管定位时间、气道峰压增加率、咽痛声嘶发生率等方面优于双腔气管插管技术,具有很好的临床应用价值。Objective To explore the application value of bronchial occluder in thoracoscope and laparoscope combined with radical resection of esophageal cancer .Methods according to the different methods of anesthesia ,50 esophageal cancer pa-tients were divided into group A（n=25） and group B（n=25）,group A adopted the bronchial obstruction technology ,and group B used double lumen endotracheal intubation technique .Effects of single lung ventilation of the 2 groups were analyzed .Results After 30-minute single lung ventilation ,PaO2 ,PaCO2 ,SaO2 and Paw of the 2 groups were compared ,there was no statistical differ-ence（P〉0.05）;the positioning time,rate of increased airway peak pressure ,incidence of sore throat and hoarseness of the 2 groups were compared,the difference was statistically significant （P〈0.05）;Operative field exposure and satisfaction rate of lung collapse were compared,there was no statistical difference（P〉0.05）.Conclusion The bronchial obstruction technology is su-perior to double lumen endotracheal intubation technique in the positioning time ,rate of increased airway peak pressure ,incidence of sore throat and hoarseness ,and it has great value in clinical application .
