结节性筋膜炎的MRI表现

摘　　要：目的观察结节性筋膜炎（NF）的MRI表现。方法回顾性分析8例经手术病理确诊的NF患者的MRI表现。结果 8例患者中,发生前臂皮下型3例,骶尾部皮下型2例,肩部皮下型、下肢肌内型和下肢肌间隙型各1例;病灶直径1.23-9.52cm。皮下型病灶T1WI均呈等信号;T2WI均表现为高信号,2例病灶周围轻度水肿;肌内型病灶T2WI呈中央高信号,周边稍低信号改变;肌间型病灶T2WI比T1WI信号低。皮下型病灶增强扫描可见明显强化的＂筋膜尾＂征;肌内和肌间型病灶增强后均呈不均匀强化,肌内型可见＂筋膜尾＂征。结论 NF病灶的MRI表现与其病理类型有关,位于皮下的病灶较具特征性,＂筋膜尾＂征具有一定的诊断价值。Objective To observe the imaging appearances of MRI in nodular fasciitis（NF）.Methods MRI data of 8 cases with NF proved by pathology after operation were retrospectively analyzed.Results In 8 cases,3 cases were occurred in subcutaneous at forearm,2cases were found in subcutaneous at sacrococcygeal region,1 case was under the skin of shoulder,1 case was intramuscularly of lower limb and 1 case was in the gap of muscles.The range of diameter was from 1.23 to 9.52 cm.Aubcutaneous NF showed isointensity signal on T1 WI,hyperintense on T2 WI,2 cases showed slightly perilesional tissue edema,intramuscular form showed higher T2 signal in the central,while slightly hypointensity in periphery.Lesion among in muscles showed lower signal on T2 WI than T1 WI.On enhancement,all lesions under the skin were obviously enhanced,and ＂aponeurosis tail＂ sign could be seen;lesion in or out of the muscle showed heterogeneity isointensity and ＂aponeurosis tail＂ sign could be found in intramuscular form.Conclusion The MRI features of NF are correlated with its pathological category.The NF lesions in subcutaneous has some characters.The ＂aponeurosis tail＂ have a certain diagnostic significance.
