树突细胞、白介素-23与溃疡性结肠炎的关系

摘　　要：目的:探讨树突细胞(dendritic cell,DC)和白介素(interleukin,IL)-23对提高溃疡性结肠炎(ulcerative colitis,UC)诊治水平的价值和意义.方法:以UC、肠易激综合征(irritable bowe syndrome,IBS)患者的肠黏膜蜡块标本为研究对象,应用免疫组织化学SP法检测两种病变肠黏膜中S100+DC、CD83+DC及IL-23P19的表达情况,并分析UC组中急性发作期与慢性迁延期,轻型组与重型组,美沙拉嗪治疗前后IL-23P19的浸润密度.结果:UC与IBS患者肠黏膜中S100+D C、CD83+DC、IL-23P19的浸润密度(个/mm2)比较,分别为27.48±9.23 vs 9.20±3.91,6.62±2.59 vs 3.20±1.59,105.97±25.30 vs 32.48±11.61,差异有统计学意义(P<0.05);UC组中,急性发作期与慢性迁延期、轻型组与重型组、美沙拉嗪治疗前后IL-23P19浸润密度(个/mm2)比较,分别为113.83±28.83 vs49.35±8.11,75.03±21.39 vs 117.38±28.32109.89±25.73 vs 47.25±8.67,差异有统计学意义(P<0.05).结论:UC患者肠黏膜中,随着DC表达的增多及分化能力增强,IL-23分泌增多;IL-23可作为UC判断疾病发作、病情轻重及疗效的监测指标.AIM：To evaluate the expression of dendritic cell（DCs） and interleukin（IL）-23 in ulcerative colitis（UC） to improve the diagnosis and treatment of UC.METHODS：Sixty UC and 60 irritable bowel syndrome（IBS） colon tissue sections were used to detect the expression of S100~＋ DCs,CD83~＋DCs and IL-23P19 by immunohistochemistry.The expression of IL-23P19 in the mucosa was compared between acute and chronic stages,between mild and severe type diseases,and between before and after mesalazine treatment in the UC group.RESULTS：There was a statistical difference in the infiltrated densities of S100~＋ DCs,CD83~＋ DCs and IL-23P19 cells between the UC group and IBS group（27.48 cells/mm^2± 9.23 cells/mm^2 vs 9.20 cells/mm^2 ± 3.91cells/mm^2,6.62 cells/mm^2 ± 2.59 cells/mm^2vs 3.20 cells/mm^2 ± 1.59 cells/mm^2,105.97cells/mm^2 ± 25.30 cells/mm^2 vs 32.48 cells/mm^2 ± 11.61 cells/mm^2,P〈0.05）.In the UC group,there was a statistical difference in the expression of IL-23P19 between acute and chronic stages,between mild and severe type diseases,and between before and after mesalazine treatment（113.83 cells/mm^2 ±28.83 cells/mm^2 vs 49.35 cells/mm^2 ± 8.11cells/mm^2,75.03 cells/mm^2 ± 21.39 cells/mm^2 vs 117.38 cells/mm^2 ± 28.32 cells/mm^2,109.89 cells/mm^2 ± 25.73 cells/mm^2 vs 47.25cells/mm^2 ± 8.67 cells/mm^2,P 〈 0.05）.CONCLUSION：In UC colon tissue,the production of IL-23P19 increases with the increase in the infiltrated density and activity of DCs.The expression of IL-23P19 may be used as an indicator for evaluation of the disease stage,the degree of pathological changes and treatment efficacy in UC.
