口腔颌面部恶性肿瘤副肿瘤性内分泌紊乱症的临床分析

摘　　要：目的探讨口腔颌面部恶性肿瘤副肿瘤性内分泌紊乱症的临床特点及意义。方法对142例口腔颌面部恶性肿瘤患者的临床资料进行回顾性研究,分析其副肿瘤性内分泌紊乱症的发病率与肿瘤组织学类型及TNM分期的关系。结果 13例(9.1%)出现副肿瘤性内分泌紊乱症,其中高钙血症8例(5.6%),白细胞增多症5例(3.5%),高钙血症伴发白细胞增多症4例(2.8%),抗利尿激素分泌异常症4例(2.8%)。13例副肿瘤性内分泌紊乱症患者中鳞癌11例(84.6%),横纹肌肉瘤1例(7.7%),腺癌1例(7.7%);TNM分期Ⅲ期7例(53.8%),Ⅳ期6例(46.2%),Ⅰ、Ⅱ期均为0例。结论口腔颌面部恶性肿瘤副肿瘤性内分泌紊乱症发生率较高,主要发生于TNMⅢ期及Ⅳ期鳞癌患者,其临床表现复杂多样,应引起重视。Objective To discuss the clinical characteristics and significances of paraneoplastic endocrine disorders in patients with oral and maxillofacial malignant tumor. Methods With a retrospective review of data from 142 patients with oral and maxillofacial malignant tumor, the morbidity,tumor histnlogic type and TNM staging of paraneoplastic endo- crine disorders were analysed and counted. Results 13 patients（9.2% ） were suffered from paraneoplastic endocrine disorders. There were 8 patients （ 5.6% ） with humoral hypercalcemia, 5 （ 3.5 % ） with leucocytosis, 4 （ 2.8% ） with hu- moral hypercalcemia and leueocytosis, 4 （2.8%） with syndrome of inappropriate antidiuretic hormone secretion （SI- A DH）. In 13 patients with paraneoplastic endocrine disorders, there were 11 cases（84.6% ） of squamous cell carcino- ma, one case（ 7.7% ） of rhabdomyosareoma, and one case（7.7% ） of adenocarcinoma; There were 7 cases（53.8% ） o（ TNM staging Ⅲ and 6 cases（ 46.2% ） of TNM staging IV, with none of TNM staging Ⅰ or Ⅱ Conclusion Paraneo- plastic endocrine disorders in patients with oral and maxillofaeial malignant tumor had relatively high morbidity and were mainly found in the eases of squamous cell carcinoma with TNM staging m or IV. They had so complicated clinical mani- festations and should be paid attention to.
