摘 要:目的探讨三维能量多普勒超声对甲状腺良恶性肿瘤的鉴别诊断价值,为临床诊断提供参考依据。方法选取收治的74例甲状腺肿瘤患者作为研究对象,按病变性质分为良性组38例和恶性组36例,比较两组患者的血管化指数(VI)、血流指数(FI)、血管化-血流指数(VFI)。结果恶性组VI、FI、VFI均高于良性组,差异有统计学意义(P<0.05);血流分级Ⅲ级VI、FI、VFI均高于Ⅱ级(P<0.05);VI≥1.168诊断甲状腺恶性肿瘤的敏感性为91.67%,准确性为79.73%,特异性为68.42%;FI≥32.408诊断甲状腺恶性肿瘤的敏感性为88.89%,准确性为70.27%,特异性为52.63%;VFI≥0.437诊断甲状腺恶性肿瘤的敏感性为88.89%,准确性为77.03%。结论恶性甲状腺肿瘤VI、FI、VFI均高于良性肿瘤,三维能量多普勒超声为甲状腺肿瘤术前鉴别诊断提供了重要依据,值得临床重视。Objective To explore the diagnostic value of three-dimensional power Doppler ultrasound in differential diagnosis of benign and malignant thyroid tumor, provids reference for the clinical diagnosis. Methods 74 cases of thyroid cancer patients from 2013 July to 2014 May were treated as the research object and were divided into the benign group (n=38 cases) and malignant group (n= 36 cases) according to the nature of the lesions, compared patients of two groups with vascular index (VI), flow index (FI), vascularization flow index (VFI). Results the group VI, FI, VFI were higher than grade Ⅱ group, the difference was statistically significant (P〈0.05); VI, FI, VFI in malignant group were higher than those in benign group, the difference was statistically significant (P〈0.05); the sensitivity of VI≥ 1. 168 in diagnosis of thyroid malignant tumor was 91.67% (33/36), the accuracy was 79.73% (59/74), the specificity was 68.42% (26/38) ; the sensitivity of FI ≥32. 408 in diagnosis of thyroid malignant tumor was 88.89% (32/36), the accuracy was 70.27% (52/74), the specificity was 52.63% (20/38) ; the sensitivity of VFI ≥0. 437 in diagnosis of thyroid malignant tumor was 88.89% (32/36), the accuracy was 77.03% (57/74). Conclusion Malignant thyroid tumor VI, FI, VFI are higher than those of benign tumor. Providing an important basis for the differential diagnosis of thyroid neoplasms, 3D power Doppler ultrasonography is worthy of clinical attention.
