Tecnis ZMB00多焦点人工晶状体的临床应用疗效分析

摘　　要：目的探讨Tecnis ZMB00多焦点人工晶状体的临床应用效果。方法对2012年3月至2013年7月行白内障摘出及人工晶状体植入的老年性白内障患者进行回顾性研究,其中多焦点组植入Tecnis ZMB00非球面多焦点人工晶状体,共28例(35眼);单焦点组植入TecnisZA9003非球面单焦点人工晶状体,共30例(35眼),对比2组患者术后1周、1个月、3个月裸眼远、近视力,矫正远、近视力及脱镜率。结果术后2组患者裸眼远视力分别为0.31±0.23、0.24±0.19,差异无统计学意义(t=1.388,P=0.170);最佳矫正远视力分别为0.22±0.16、0.20±0.12,差异无统计学意义(t=0.592,P=0.556);最佳矫正近视力分别为0.46±0.23、0.52±0.23,差异无统计学意义(t=1.091,P=0.279)。与单焦点组比较,多焦点组裸眼近视力、最佳矫正远视力下的近视力有明显优势,差异均有统计学意义(均为P<0.01)。多焦点组术后脱镜率82.8%(29/35),明显高于单焦点组5.7%(2/35),差异有统计学意义(P<0.01)。结论Tecnis ZMB00多焦点人工晶状体可以提供良好的近视力,提高脱镜率,从而改善患者术后的生活质量。Objective To evaluate the safety and effectiveness of the aspheric diffractive Tecnis multifocal ZMB00 intraocular lens（ IOL）. Methods Subjects underwent implantation with the ZMB00 IOL or the aspheric monofocal Tecnis ZA9003 IOL were enrolled in this study,including 28 cases（ 35 eyes） with ZMB00 IOL and 30 cases（ 35 eyes） with ZA9003 IOL,The uncorrected near and distant visual acuity,corrected near and distant visual acuity,and spectacle independence at postoperative 1 week,1 month and 3 months between two groups were compared.Results Mean uncorrected distant visual acuity of two groups were 0. 31±0. 23 and 0. 24±0. 19,respectively,there was no statistical difference（ t = 1. 388,P = 0. 170）. Mean best corrected distant visual acuity of two groups were 0. 22±0. 16 and 0. 20±0. 12,respectively,there was no statistical difference（ t = 0. 592,P= 0. 556）. Mean best corrected near visual acuity of two groups were 0. 46±0. 23 and 0. 52±0. 23,respectively,there was no statistical difference（ t = 1.091,P= 0. 279）. There were significant difference in near visual acuity of uncorrected distant visual acuity and best corrected near visual acuity between two groups（ P〈0. 01）. The proportions of spectacle independence in ZMB00 IOL group and ZA9003 IOL group were 82. 8%（ 29 /35） and 5. 7%（ 2 /35）,there was significant difference（ P〈0. 01）. Conclusion ZMB00 IOL can provide better near visual acuity and high spectacle independence to improve the quality of life.
