2D PC法头颅MRA显示椎动脉颅内段狭窄与眩晕相关性分析

摘  要:目的探讨眩晕与椎动脉颅内段狭窄的关系。方法 2010年11月-2012年7月共343例以头晕为主诉的患者行头颅MRA检查,MRA均采用3DTOF及2D PC序列分别显示颈内动脉颅内段及其分支、椎动脉颅内段及其分支情况,详细追踪患者病史,记录患者头晕是否伴视物旋转、是否与体位改变有关。症状1为单纯头晕(dizziness);症状2为头晕伴视物旋转;症状3为头晕并与体位改变有关;症状4为头晕伴视物旋转并与体位改变有关。症状2、症状3、症状4均视为眩晕(Vertigo)。运用SPSS 16.0软件进行数据统计学处理,分析眩晕发生与椎动脉颅内段狭窄的相关性。结果 343例患者中单侧椎动脉颅内段狭窄149例,其中右侧狭窄89例,左侧狭窄60例;双侧椎动脉颅内段狭窄17例;单侧椎动脉颅内段闭塞37例,其中右侧闭塞22例,左侧闭塞15例;双侧椎动脉颅内段闭塞0例;基底动脉迂曲及基底动脉尖上抬32例;基底动脉狭窄41例;基底动脉闭塞0例。在343例患者中共有40例基底动脉异常患者,不同症状与基底动脉异常率无差异,P=0.532。343例患者中眩晕患者183例,其中椎动脉颅内段狭窄者113例,眩晕与椎动脉颅内段狭窄有关联性,关联系数0.275,P=0.000;椎动脉颅内段闭塞患者25例,眩晕与椎动脉颅内段闭塞无关联性,P=0.067。椎动脉颅内段迂曲患者37例,眩晕与椎动脉颅内段迂曲无关联性,P=0.845。双侧椎动脉颅内段不对称患者3例,眩晕与双侧椎动脉颅内段不对称无关联性,P=1.000。结论 2D PC法MRA能够准确评估双侧椎动脉颅内段狭窄情况,椎动脉颅内段狭窄与临床眩晕发生密切相关。Objective To determine the relationship between vertigo and stenosis of vertebral artery in skull shown by 2D PC head MRA. Methods 2D PC MRA of the head were performed in a total of 343 subjects who had vertigo or dizziness as an isolated symptom from Nov 2010 to Jul 2012. Correlation between vertigo and vertebral artery stenosis was assessed with SPSS 16.0 software. Results Stenosis of mono-vertebral artery was 149 cases,and stenosis of right vertebral artery was 89 cases, stenosis of left vertebral artery was 60 cases, stenosis of bilateral vertebral artery was 17 cases. Occlusion of mono-vertebral artery was 37 cases,occlusion of right vertebral artery was 22 cases,occlusion of left vertebral artery was 15 cases, occlusion of bilateral vertebral artery was 0 case. Of the 343 patients, there were 183 vertigo cases, and vertebral artery stenosis was 113 cases of these patients, P=0.000. Conclusion MR angiography providing angiogram-like images of the intracranial vessels may sometimes avoid invasive angiography. It is accurate to evaluate cranial vertebral artery stenosis by using 2D PC MRA of the head, and vertigo onset is related to vertebral artery stenosis closely.
