治疗性内镜辅助下ERCP术对胆源性胰腺炎并发症的影响研究

摘　　要：目的:探讨治疗性内镜辅助下ERCP术对胆源性胰腺炎并发症的影响。方法:选取我院胆源性胰腺炎患者116例,采用随机数字表法将患者分为观察组和对照组,对照组(57例)患者采用常规基础治疗,观察组(59例)患者采用治疗性内镜辅助下逆行胰胆管造影(ERCP)术治疗,比较两组患者治疗效果和并发症发生率的差异。结果:治疗前,两组APACHEⅡ评分、Ranson评分比较差异无统计学意义(P>0.05),治疗后,观察组上述指标均明显低于对照组(P<0.05),治疗前,两组血清CRP水平比较差异无显著性(P>0.05),治疗后,观察组CRP水平明显低于对照组和同组治疗前(P<0.05),且观察组血清淀粉酶恢复时间、腹痛缓解时间和住院时间明显低于对照组,两组比较具有明显差(P<0.05),观察组并发症(胆瘘、胆道出血、肝肾衰竭、假性囊肿、肠梗阻和胰周脓肿)发生率明显低于对照组(P<0.05)。结论:治疗性内镜辅助下ERCP术在改善胆源性胰腺炎治疗效果的同时明显降低术后并发症的发生风险。Objective： To investigate the ERCP surgery for biliary pancreatitis. Method effect study on complications in therapeutic endoscopy assisted ： 116 patients with biliary pancreatitis patients in our hospital, were randomized divided into observation group and control group, the control group （57 cases） were given conventional treatment, the observation group （59 cases ） were treated by endoscopic assisted endoscopic ret- rograde cholangiopancreatography （ ERCP ） , compared the effect of two groups and the incidence of compli- cations difference. Result. Before treatment, APACHE score, Ranson score of the two groups was not statis- tically significant （P〉0.05 ）, after treatment, the above index were significantly lower than the control group （ P〈0.05） , before treatment, the difference serum CRP level of the two groups was not significant （P〉0. 05）, after treatment, the observation group CRP was significantly lower than the control group （P〈0.05 }, The serum amylase in abdominal pain relief, recovery time and hospitalization time of the observation group was significantly lower than the control group, There＇s significant difference （P〈0.05 ）. The complication rate of observation group was significantly lower than control group （ P〈0.05 ）. Conclusion： The therapeutic endoscopic assisted ERCP operation improves the effect of biliary pancreatitis and decrease the risk of postop- erative complications.
