中枢神经系统神经元肿瘤的MRI特点分析

摘　　要：目的探讨中枢神经系统神经元肿瘤的MRI特点。方法回顾性分析24例病理诊断为中枢神经系统神经元肿瘤的患者资料,MRI采用自旋回波T1WI、快速自旋回波(TSE)T2WI、液体衰减反转恢复(FLAIR)、T1WI增强扫描,观察其MRI特点包括病灶位置、形状、信号特点、占位效应及增强表现。结果 24例中胚胎发育不良性神经上皮瘤15例,节细胞胶质瘤7例,中枢神经细胞瘤2例,15例胚胎发育不良性神经上皮瘤中,位于颞叶12例、额叶2例,顶枕叶1例。MRI上病灶主要位于皮质区,肿瘤表现为单发或多发囊性改变,MRI表现为T1WI低信号,FLAIR和T2WI高信号,增强扫描12例病灶不强化,3例可见斑片、结节、环形强化,病灶周围均未见水肿。7例节细胞胶质瘤中,颞叶5例,额叶1例,顶叶1例,MRI表现为囊实混合性肿块,T1WI上为低信号,T2WI/FLAIR为高信号,其中2例伴有钙化,无瘤周水肿,增强后实性结节明显强化。中枢神经细胞瘤2例,位于侧脑室内,MRI呈囊实性不规则异常信号,T1WI等低信号,T2WI/FLAIR等高混杂信号,增强后实性部分及分隔明显强化。结论中枢神经系统神经元肿瘤的MRI表现具有一定的特点,有助于与其他肿瘤的鉴别,掌握这些特点,并结合临床,可以对其作出正确的诊断。Objective To explore the MRI features of various neuronal tumors of the central nervous system（CNS）.Methods Twenty-four patients with pathology proved neuronal tumors of CNS were collected.MRI examinations were performed including T1 WI,T2WI,FLAIR and T1 WI enhancement.MRI features including lesion location,shape,signal intensity,mass effect and contrast enhancement were reviewed retrospectively.Results Of the 24 patients with CNS neuronal tumors,there were dysembryoplastic neuroepithelial tumor（DNET,15patients）,ganglioglioma（7patients）and central neurocytoma（2patients）.In 15 DNET cases,the lesion was located in different lobes（12temporal lobe,3frontal lobe and 1parietal-occipital lobe）,mainly involved cortical area.The lesion presented solitary or multiple cystic appearances.Hyperintensity on T2WI/FLAIR and hypointensity on T1 WI was revealed.No Contrast enhancement was noted on twelve cases.The other 3case showed patchy,nodular and circular enhancement.No edema was found around the lesions.Seven cases with ganglioglioma included 5temporal lobe,1frontal lobe,and 1parietal lobe.MRI features were cystic-solid tumors,Hyper-isointensity on T2WI/FLAIR and hypo-isointensity on T1 WI was revealed.Two cases showed calcification.No edema was found around the tumors.The solid nodular portion of the tumors showed obvious enhancement after contrast administration.Two patients with central neurocytoma were located in lateral ventricles.Cystic-solid irregular abnormal signal was found on MRI,which showed hypo-isointensity on T1 WI and hyper-isointensity on T2WI/FLAIR.The solid and septum portion of the tumor presented obvious enhancement.Conclusion Neuronal tumors in CNS are characteristics on MRI,which are helpful to differentiate these diseases from other brain tumors.By mastering the imaging and clinical features,we can make the correct diagnosis of the tumors.
