年龄相关性白内障角膜前表面高阶像差分析

摘　　要：目的观察年龄相关性白内障患者角膜前表面高阶像差的大小及其与年龄的关系。方法对2013年1月至12月在我院行手术治疗的年龄相关性白内障患者200例（200眼）术前相关资料进行回顾性分析,根据年龄将患者分为4组：组1：≥50~59岁（34例）;组2：≥60~69岁（58例）;组3：≥70~79岁（66例）;组4：≥80岁（42例）。采用iTrace视觉功能分析仪测量角膜前表面6mm直径内总高阶像差、彗差、三叶草、球差,采用方差分析和Pearson相关分析对获得的数据进行统计。结果200例（200眼）患者角膜前表面总高阶像差、彗差、三叶草、球差值分别为0.268~0.897（0.534±0.122）μm、0.024~0.726（0.287±0.119）μm、0.019~0.681（0.276±0.121）μm和0.054~0.552（0.268±0.083）μm。4组患者角膜前表面总高阶像差、彗差、三叶草值随年龄的增大而增大,差异有统计学意义（P〈0.001）,且两两比较差异均有统计学意义（均为P〈0.05）;4组患者角膜前表面球差差异无统计学意义（P=0.297）。4组患者的球镜值、角膜散光、角膜Q值差异均无统计学意义（均为P〉0.05）。角膜前表面总高阶像差、彗差、三叶草值与年龄呈正相关（r=0.274、0.203、0.188,均为P〈0.001）,但球差与年龄无明显相关性（r=0.034,P=0.715）。结论年龄相关性白内障患者角膜前表面高阶像差在不同个体间差异很大,总高阶像差、彗差、三叶草像差随年龄增长逐渐增大,球差随年龄增长无明显变化。Objective To investigate the distribution of anterior corneal higher-order aberrations（ HOA） in age-related cataract and their correlations with age. Methods Retrospective study. Two hundred patients with age-related cataract in our hospital from January 2013 to December2013 were divided into four groups according to their age： group 1（ 50- 59 years old,34 cases）,group 2（ ≥60- 69 years old,58 cases）,group 3（ ≥70- 79 years old,66 cases）,group 4（ ≥80 years old,42 cases）. The total HOA,coma aberration,trefoil aberration and spherical aberration in the central 6-mm zone of the anterior cornea were measured with iTrace wavefront analyzer. ANOVA and Pearson analysis were used to analyze the data. Results For all the cases,the total HOA,coma aberration,trefoil aberration and spherical aberration were 0.268- 0. 897（ 0. 534 ± 0. 122） μm,0. 024- 0. 726（ 0. 287 ± 0. 122） μm,0. 019- 0. 681（ 0. 276± 0. 121） μm and 0. 054- 0. 552（ 0. 268 ± 0. 083） μm,respectively. The total HOA,coma aberration,trefoil aberration in four groups were increased with the increase of age（ P〈0. 001）,and there were statistical differences between each two groups（ all P〉0. 05）. There was no statistical difference in spherical aberration among four groups（ P = 0. 297）. There was no statistical difference in spherical value,corneal astigmatism and Q value among four groups（ all P〈0.05）. The positive correlations were found between age and total HOA,coma aberration and trefoil aberration（ r = 0. 274,0. 203,0. 188; all P〈0. 001）. However,no correlation was found between age and spherical aberration（ r = 0. 034,P = 0. 715）. Conclusion Anterior corneal higher-order aberrations in people with age-related cataract have greatly differences among subjects.With the increase of age,total HOA,coma aberration and trefoil aberration increased,whereas spherical aberration do not change.
