同期放化疗联合辅助化疗治疗鼻咽癌患者的临床观察

摘　　要：目的探讨同期放化疗联合辅助化疗治疗鼻咽癌患者的疗效及不良反应。方法选择2010年1月至2014年1月间收治的76例初治鼻咽癌患者,按照随机数字表法分为研究组和对照组,每组38例。对照组患者采用同期放化疗方案治疗,研究组患者应用同期放化疗联合辅助化疗治疗。观察两组患者的临床疗效以及化疗引起的不良反应。结果研究组患者的总有效率为73.7%,对照组为44.7%,研究组显著高于对照组,差异有统计学意义（P〈0.05）。两组主要不良反应多数为Ⅰ-Ⅱ度,研究组脱发、口腔黏膜炎发生率显著高于对照组,差异有统计学意义（P〈0.05）,两组恶心、呕吐、腹泻、外周神经毒性、肝损伤发生率比较,差异无统计学意义（P〉0.05）。结论同期放化疗联合辅助化疗治疗Ⅲ-Ⅳ期鼻咽癌患者可以显著提高临床疗效,但口腔黏膜炎和脱发的发生率较高,临床治疗应予以注意。Objective To explore the curative effect and toxicity of concurrent chemoradiotherapy plus adjuvant chemotherapy in the treatment of nasopharyngeal carcinoma patients. Methods From January2010 to January 2014 76 untreated nasopharyngeal carcinoma patients in The Affiliated Hospital of Guangdong Medical College were enrolled in the study and randomly divided into the study group and the control group,40 cases in each group. The patients in the control group accepted concurrent chemoradiotherapy regimens,and those in the research group accepted concurrent chemoradiotherapy plus adjuvant chemotherapy. The curative effect and the side effects between the two groups were observed. Results The total effective rate of the study group was 73. 7%,and the control group was 44. 7%; the total efficiency of study group was significantly higher than that of control group,with significant difference（ P〈0. 05）. The main toxicity was mainly Ⅰ - Ⅱ degree. The incidence of hair loss,oral mucositis of the study group were significantly higher than those of in control group（ P〈0. 05）. Nausea and vomiting,diarrhea,peripheral neurotoxicity,liver injury incidence rate between tht two groups had no significant differences（ P〉0. 05）. Conclusions Concurrent chemoradiotherapy plus adjuvant chemotherapy in the treatment of stage Ⅲ - Ⅳ nasopharyngeal carcinoma patients can significantly achieve good curative effect. But it can also cause a high incidence of oral mucositis and alopecia and should be paid attention.
