摘 要:目的探讨超声造影与核磁共振(MRI)增强方式诊断乳腺病变的特点及异同。方法选取2010年11月至2014年2月间收治的经手术或穿刺活检病理诊断为乳腺病变的64例患者(共73个病灶),分析超声造影与MRI增强扫描方式的声像特征,比较二者的异同点。结果超声造影与MRI增强均匀性不完全一致(P=0.077),且超声造影(39例)较MRI(24例)增强扫描表现出更多的不均匀增强(P=0.032)。而超声造影和MRI增强后显示病变面积的一致性较好(P=0.010),且两者差异无统计学意义(P=0.338);但超声造影技术灌注缺损区面积大于MRI增强扫描,且两者差异有统计学意义(P=0.013);不均匀增强的病变面积大于均匀增强的病变面积(P<0.05)。超声造影与MRI增强时间-强度曲线类型无明显相关性。结论超声造影与MRI增强方式在乳腺病变的诊断中存在一定的相同点,但由于二者的原理不同,也存在一定的差异,临床诊断应根据二者的原理进行区分应用。Objective To explore the performance characteristics and similarities and differences of breast lesions with ultrasound contrast and enhanced magnetic resonance methods. Methods 64 cases of patients with breast lesions( 73 lesions) diagnosed by biopsy pathology in Department of Radiology Second Affiliated Hospital,Xin-jiang Medical University from November 2010 to February 2014 were chosed as research data,and the sonographic feature of ultrasound and enhanced MRI scan were compared. Results The Kappa test showed that there was not completely consietent between the ultrasound and enhanced MRI scan method( P = 0. 077),and there was more heterogeneous of the ultrasound than that of the enhanced MRI scan methods( 39 cases vs 24 cases)( P = 0. 032). The Kappa test showed that there was good consietent between the ultrasound and enhanced MRI scan method( P = 0. 010). The perfusion defect area of ultrasound imaging was larger than that of the enhanced MRI scans( P = 0. 013). And the inhomogeneous enhancement area of the lesion area was greater than that of the homogeneous enhancement area( P〈0. 05).There was no significant correlation of enhancement time-intensity curve type betweent the two methods.Conclusions There are some same points of the ultrasound and MRI contrast enhancement for the diagnose of breast lesions,while based on the principle differences between the two groups,there are some differences,the clinical diagnosis should be based on two principles to distinguish for applications.
