摘  要:目的通过研究膜型基质金属蛋白酶-1(membrane type matrix metalloproteinase-1,MT1-MMP)及基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)在甲状腺乳头状癌组织及癌旁组织中的表达,探讨其表达在甲状腺乳头状癌发生发展中的意义。方法采用免疫组织化学SP法检测164例甲状腺乳头状癌及其癌旁组织标本中MT1-MMP和MMP-2的表达,分析二者的表达与甲状腺乳头状癌临床病理特征间的关系。结果在甲状腺乳头状癌癌旁组织中,MT1-MMP表达阳性率为11.0%(18/164),MMP-2表达阳性率为14.0%(23/164);在甲状腺乳头状癌组织中,MT1-MMP及MMP2的表达阳性率分别为61.6%(101/164)和67.7%(111/164);二者在癌组织中的表达阳性率均高于在癌旁组织中的表达阳性率(P<0.05)。癌组织中MT1-MMP和MMP-2的表达与甲状腺乳头状癌的淋巴结转移相关(P<0.05),MMP-2的表达还与甲状腺乳头状癌的包膜侵犯相关(P<0.05);MT1-MMP和MMP-2在甲状腺乳头状癌组织中的表达呈正相关(r=0.256,P<0.05)。结论 MT1-MMP和MMP-2可能参与了甲状腺乳头状癌的包膜侵犯和淋巴结转移,且MT1-MMP和MMP-2在甲状腺乳头状癌的进展过程中可能发挥协同作用。Objective To investigate expressions and biological function of membrane type matrix metalloproteinase-1(MT1-MMP) and matrix metalloproteinase-2(MMP-2) in papillary thyroid carcinoma. Methods The expressions of MT1-MMP and MMP-2 in 164 cases of papillary thyroid carcinoma and paracancerous tissues were detected by immunohistochemistry. The association between the expressions of MT1-MMP and MMP-2 and clinicopathological characteristics of papillary thyroid carcinoma was analyzed. Results In paracancerous tissues, the positive expression rate of MT1-MMP was 11.0%(18/164), and the positive expression rate of MMP-2 was 14.0%(23/164). In papillary thyroid carcinoma tissues, the positive expression rates of MT1-MMP and MMP-2 was 61.6%(101/164) and 67.7%(111/164), respectively. The expressions of MT1-MMP and MMP-2 in carcinoma tissues and para carcinoma tissues were statistically significant differences(P〈0.05). The expressions of MT1-MMP and MMP-2 in papillary thyroid carcinoma tissues correlated with the lymph node metastasis(P〈0.05). In addition, the expression of MMP-2 in papillary thyroid carcinoma tissues correlated with capsule invasion(P〈0.05). The positive correlation was found between the expressions of MT1-MMP and MMP-2 in papillary thyroid carcinoma tissues(r=0.256, P0. 05). Conclusions MT1-MMP and MMP-2may be involved the thyroid capsule invasion and lymph node metastasis of papillary thyroid carcinoma. MT1-MMP and MMP-2 may be involved in the progression of papillary thyroid carcinoma.
