摘 要:目的探讨单指数ADC值及扩散峰度成像在肝纤维化大鼠模型中的应用。方法取50只SD大鼠建立肝纤维化模型,作为模型组,另取15只SD大鼠作为正常对照组。对模型组于建模后第14、21、28、35、42、49、56、63、70天各随机抽取5只(建模过程中死亡5只),行3.0T MRI及病理检查,对正常对照组直接行MRI及病理检查,获得肝脏ADC值、Dapp、Kapp值及肝纤维化病理分期。并进行统计学分析。结果大鼠不同肝硬化病理分期间ADC、Dapp及Kapp值差异均有统计学意义(P均<0.001)。ADC、Dapp值与肝纤维化病理分期呈负相关(r=-0.79、-0.73,P均<0.001),Kapp值与肝纤维化病理分期呈正相关(r=0.59,P<0.001)。结论扩散峰度成像可以作为一种无创性的新技术,单指数ADC值结合扩散峰度成像可较为全面、准确地反应肝纤维化过程中病理生理及组织微观结构的改变。Objective To explore the application value of mono-exponential ADC values and diffusion kurtosis imaging(DKI)for liver fibrosis in rat models.Methods Liver fibrosis models were established in 50 SD rats as the study group.Meanwhile,another 15 normal SD rats were treated as control group.Totall 5rats were dead during the modeling.MR scan and histopathological examination were performed on 14,21,28,35,42,49,56,63,70 days after modeling.Five rats were killed and examed on each point in study group.Rats in control group were direct killed after MR scan.The ADC,Dapp,Kappand pathological stage were obtained.And statistical analysis was done.Results There were statistical differences of ADC,Dappand Kappin various pathological stages of liver fibrosis(all P〈0.001).ADC and Dappwere negatively correlated with pathological stags(r=-0.79,-0.73,both P〈0.001).Positive correlation was found between Kappand pathological stages(r=0.59,P〈0.001).Conclusion DKI can be used as a new noninvasive imaging method.DKI and mono-exponential ADC values can reflect the changes of liver fibrosis in pathophysiology and microstructure considerately.
