年龄相关性白内障术前角膜屈光状态与非球面性特征及相互关系分析

摘　　要：目的分析年龄相关性白内障患者术前角膜屈光状态及非球面性的特征及相互关系。方法临床病例系列研究。收集2012年11月至2014年2月在郑州大学第一附属医院眼科就诊的40～90岁年龄相关性白内障患者404例（558只眼），记录术前角膜的曲率、散光度数、散光轴位和Q值，从年龄、性别两方面行相关性分析，并分析角膜屈光状态及非球面性的相关性。结果患者术前角膜曲率与年龄比较差异无统计学意义（r=0．015，P=0．75），性别间比较差异无统计学意义（r=0．013，P=0．56）。角膜散光与年龄比较差异有统计学意义（r=0．827，P=0．003），角膜散光与性别比较差异无统计学意义（r=0．008，P=0．78）。散光度数≥1．0D的占43．73％（244／558）。随年龄增长，顺规性散光减少，逆规性散光增多，斜轴散光无明显变化。角膜Q值与年龄、性别比较差异无统计学意义（r=-0．021，P=0．75）、（r=0．014，P=0．78）。角膜曲率、散光度数及Q值三者比较差异无统计学意义（P〉0．05）。结论年龄相关性白内障患者术前角膜曲率、散光度数及Q值间无相关性。随年龄增长，角膜散光增大，逆规性散光为主，约半数患者需术中矫正散光。Objective To study the characteristics and correlations of corneal refractive status and asphericity in age-related cataract patients before operation. Methods A total of 404 age-related cataract patients （558 eyes） hospitalized in our hospital was collected from November 2012 to Febru- ary 2014 and recorded with preoperative corneal curvature, astigmatism value, axis, and Q value. Da- ta were analyzed respectively with age and sex, and the correlation between corneal refractive status and value of the asphericity and those two factors. Results Preoperative corneal curvature of all sub- jects hadn＇t a correlation with age （r =0.015, P =0.75） and sex （r =-0.013, P =0.56）. Corneal astig- matism had a significant positive correlation with age （r =0.827, P =0.003）, but no correlation with sex （r =-0.008, P =0.78）. Those of astigmatism values ≥ 1.0D accounted for 43.73% （244/558）. With age, astigmatism with rule decreased, astigmatism against the rule increased while oblique astigma- tism had no obvious change. Corneal Q value hadn＇t a correlation with age （r =-0.021, P =0.75） and sex （r =0.014, P =0.78）. Corneal curvature, corneal astigmatism and corneal Q value had no cor- relation between each other. Conclusions There is no correlation among corneal curvature, astigma- tism and Q value of age-related cataract patients. Corneal astigmatism values and astigmatism against the rule increased with age. Roughly 50% need correct astigmatism during operation.
