有晶状体眼后房型人工晶状体植入矫正高度近视及散光疗效观察

摘　　要：目的探讨有晶状体眼后房型人工晶状体植入矫正高度近视及散光的临床应用价值。方法纳入2010年3月至2013年1月间14例（25只眼）高度近视及散光患者，其中男6例，女8例，年龄18～42岁，平均26．7岁。所有患者接受有晶状体眼后房型人工晶状体植入治疗，术后随访3个月，检查裸眼视力、最佳矫正视力、屈光度、散光度，并在裂隙灯下检查拱高和轴向移位等。结果所有患者术后裸眼视力较术前提高，术后3个月均达到或超过术前最佳矫正视力。术前等效球镜为-7．00～-22．0D，平均一（12．52±2．50）D，术后等效球镜为-0．25～-0．75D，平均-（0．54±0．11）D，术后屈光度较术前明显降低，差异有统计学意义（P〈0．05）。散光由术前-（1．83±1．12）D下降至术后-（0．55±0．21）D，差异有统计学意义（P〈0．05）。手术前后眼压、角膜内皮细胞密度差异无统计学意义（P〉0．05）。术后无拱高偏大者，1只眼拱高偏低。术后93．75％（15／16）的TICL患者轴向偏转〈10°。结论有晶状体眼后房型人工晶状体植入术矫正高度近视及散光可以获得良好的裸眼视力，为高度近视患者提供一种新的选择，但其长期稳定性及远期并发症需要进一步观察。Objective To investigate the efficacy and safety of the implantation of posterior chamber phakic intraocular lens （ICL） for high myopia with or without astigmatism. Methods From March 2010 to January 2013, 14 high myopia patients （25 eyes） with or without astigmatism including 6 males and 8 females were admitted to our department. The average age of these pa- tients was 26.7 years old, ranging from 18 to 42. All of them were treated by implantation of poste- rior chamber phakic ICL. Vision and diopter, as well as the results of slit lamp examination were used to evaluate the efficacy during the follow up. Results All patients were implanted ICL suc- cessfully and adhered to follow-up. The uncorrected visual acuity of every eye after surgery had im- proved, same or better than the pre-operative best corrected visual acuity. Diopter and astigmatism were both decreased after surgery, from-（12.52±2.50）D to-（0.54±0.11）D and from-（1.83±l.12）D to-（0.55±0.21）D. There was no difference in intraocular pressure and corneal endothelium before and after surgery. The low vaulting occurred in one patient. 93.75%（15/16） of TICL patients had the intended axis divertion 〈10°. Conclusions Posterior chamber phakic intraocular lens （ICL） is an ef- fective way to cure high myopia with or without astigmatism. However, the long-term stability of vi- sual acuity and complication need further observation.
