HER-2、BRCA1和BRCA2联合检测在新疆维、汉族妇女乳腺癌诊断中的应用

摘　　要：目的研究HER-2、BRCA1和BRCA2联合检测在新疆维吾尔族、汉族妇女乳腺癌早期诊断中的应用。方法通过组织芯片技术及免疫组织化学方法检测68例新疆维吾尔族散发性乳腺癌患者(观察组)、68例汉族散发性乳腺癌患者(对照组)乳腺癌组织中HER-2、BRCA1及BRCA2的表达。比较分析两者表达特点及与临床病理参数间的关系。结果观察组中HER-2的表达阳性率为41.18%(28/68),BRCA1表达阳性率为63.24%(43/68),BRCA2的表达阳性率为57.35%(39/68),均显著高于对照组的22.06%(15/68),23.53%(16/68),26.50%(18/68),差异均有统计学意义(均P<0.05)。观察组中乳腺癌组织中HER-2、BRCA1及BRCA2的表达不具有相关性;对照组中,若HER-2阴性表达时,BRCA1与BRCA2的表达具有相关性(r=0.344,P=0.002);若HER-2阳性表达时,BRCA1与BRCA2的表达不具有相关性(r=0.156,P=0.488)。新疆维吾尔族妇女BRCA1、HER-2表达与年龄、腋淋巴结转移情况、临床分期有相关性(P<0.05),HER-2、BRCA2表达与肿瘤大小有相关性(P<0.05)。结论新疆维吾尔族与汉族妇女散发性乳腺癌组织中HER-2、BRCA1及BRCA2的表达均存在差异。Objective To investigate the expression of HER-2,BRCA1 and BRCA2 in Han and Uygur patients with breast cancer in Xinjiang. Methods Expression of HER-2,BRCA1 and BRCA2 was detected by tissue microarray technique and immunohistochemical method in 68 Uygur patients and68 Han patients with sporadic breast cancer from April 2010 to April 2013. The correlation of HER-2,BRCA1 and BRCA2 expression with clinicopathological parameters of disease was analyzed.Results The positive expression rates of HER-2,BRCA1 and BRCA2 in Uygur group were 41. 18%（ 28 /68）,63. 24%（ 43 /68） and 57. 35%（ 39 /68）,respectively,which were significantly higher than those in Han group（ 22. 06%,5 /68; 23. 53%,6 /68 and 26. 50%,18 /68,all P〈0. 05）. The expression of HER-2,BRCA1 and BRCA2 was not correlated with each other in Uygur group. In Han group when HER-2 was positively expressed,the expression of BRCA1 was significantly correlated with BRCA2（ r = 0. 344,P = 0. 002）,but the correlation was not found when HER-2 was negative（ r = 0. 156,P = 0. 488）. In Uygur group the expression of HER-2 and BRCA1 was correlated with age,axillary lymph node metastasis and clinical stages（ P〈0. 05）,the expression of HER-2 and BRCA2 was correlated with tumor size（ P〈0. 05）. Conclusion The expressions of HER-2,BRCA1 and BRCA2 are different between Uygur and Han patients with breast cancer in Xinjiang.
