摘 要:目的观察基础治疗联合活血通络药物或凉血止血药物对晚期视网膜中央静脉阻塞(Central Retinal Vein Occlusion,CRVO)视力和静脉循环时间的影响。方法 167例(167眼)临床确诊为晚期CRVO的患者分为非缺血型90眼和缺血型77眼,按基础治疗联合活血药还是止血药分为活血组和止血组。治疗至少1个月以上,观察治疗前后视力和静脉循环时间。结果 (1)缺血型CRVO视力(3.451±0.714)低于非缺血型(4.09±0.586),静脉循环时间(23.523±12.162)秒较非缺血型(18.692±9.99)秒长,差异具有统计学意义。(2)晚期非缺血型CRVO患者,活血组治疗后视力由(4.017±0.575)提高至(4.262±0.511),止血组治疗后视力由(4.171±0.593)提高至(4.398±0.423),差异具有统计学意义。晚期缺血型CRVO患者,活血组治疗后视力由(3.418±0.669)提高至(3.609±0.707),差异具有统计学意义,止血组治疗后视力由(3.5±0.785)提高至(3.67±0.959),差异无统计学意义。(3)晚期非缺血型CRVO患者,活血组治疗后静脉循环时间由(18.564±9.06)秒延长至(18.813±10.919)秒,止血组治疗后静脉循环时间由(18.016±8.561)秒延长至(18.95±10.504)秒,差异均无统计学意义。(4)晚期缺血型CRVO患者,活血组治疗后静脉循环时间由(22.347±8.315)秒延长至(25.257±11.323)秒,差异无统计学意义,止血组治疗后静脉循环时间由(25.405±16.679)秒延长至(31.55±15.729)秒,差异有统计学意义。结论对于晚期非缺血型CRVO患者,在基础治疗上无论联合止血法还是活血法均可改善视力,而对于晚期缺血型CRVO患者而言,在基础治疗上联合活血法可改善视力,采用止血法使静脉循环时间延长,不利于视力改善。OBJECTIVE To observe the vision and vein circulation time of central retinal vein occlusion(CRVO) with two different methods which was general treatment combined drugs of promoting blood circulation or drugs of cooling blood and hemostasis. METHODS According to the drugs of promoting blood circulation or drugs of cooling blood and hemostasis,167 cases of advanced CRVO which is including 90 cases Non-ischemic CRVO and77 cases ischemic CRVO were divided into two groups. The treatment took at least for a month. We observed the visual activity and the vein circulation time. RESULTS 1. The vision of ischemic CRVO(3.451±0.714) was inferior to Non-ischemic CRVO(4.09±0.586)(P〈0.05),the vein circulation time of ischemic CRVO(23.523±12.162) was longer than Non-ischemic CRVO(18.692±9.99)(P〈0.05). 2. The vision was improved from(4.017±0.575) to(4.262±0.511) for Non-ischemic CRVO using the drugs of Promoting blood circulation(P〈0.05),and the vision was improved from(4.171±0.593)to(4.398±0.423)for Non-ischemic CRVO using the drugs of cooling blood and hemostasis(P〈0.05). The vision were improved from(3.418±0.669) to(3.609±0.707)for ischemic CRVO using the drugs of promoting blood circulation(P〈0.05),and the vision were improved from(3.5 ±0.785)to(3.67 ±0.959)for ischemic CRVO using the drugs of cooling blood and hemostasis(P〉0.05). 3.The vein circulation time was prolonged from(18.564±9.06) seconds to(18.813±10.919) seconds for Non-ischemic CRVO using the drugs of promoting blood circulation(P〉0.05),and the vein circulation time was prolonged from(18.016±8.561) seconds to(18.95±10.504) seconds for Non-is-chemic CRVO using the drugs of cooling blood and hemostasis(P〉0.05). The vein circulation time was prolonged from(22.347±8.315) seconds to(25.257±11.323) seconds for ischemic CRVO using the drugs of promoting blood circulation(P〉0.05),which presented no statistical significance,and
