偏头痛与颈源性头痛临床特点分析及头痛方的疗效观察

摘　　要：目的探讨偏头痛与颈源性头痛的临床特点及头痛方对两种头痛的治疗效果。方法对37例偏头痛患者和40例颈源性头痛患者的临床资料进行分析,两组患者均接受头痛方治疗,3个月后评估临床疗效。结果两种头痛患者在年龄、病程、枕部头痛、搏动性头痛及恶心、呕吐、畏光、畏声方面比较,差异均具有统计学意义(P<0.05);在性别、单双侧头痛、颞部头痛、前额头痛、顶部头痛、头痛程度方面比较,差异均无统计学意义(P>0.05)。头痛方对两种头痛的疗效相似(P>0.05)。结论偏头痛发病年龄较颈源性头痛早,多为搏动性疼痛,多伴随恶心、呕吐、畏光、畏声;偏头痛多见于颞部,颈源性头痛多见于枕部、颞部。具祛风、除湿、化瘀、补虚之功的头痛方对偏头痛和颈源性头痛的临床疗效相似。Objective To perform a comparative analysis of the clinical characteristics and response to a headache prescription between migraine and cervicogenic headache（CGH）.Methods The clinical data of 37 patients with migraine and 40 patients with CGH were analyzed.Both groups were treated with a headache prescription for three months.The treatment outcomes of the two groups were compared.Results There were significant differences between migraine and CGH patients with respect to age of onset,course of disease,throbbing pain,occipital pain,and symptoms like nausea,vomiting,photophobia,and phonophobia（P〈0.05）.However,no significant differences were noted between migraine and CGH patients in terms of sex,unilateral and bilateral involvement,temporal headache,frontal headache,parietal headache,and severity of headache（P〉0.05）.Patients with migraine or CGH responded similarly to the headache prescription（P〉0.05）.Conclusion Migraine has an earlier onset than CGH,and throbbing pain and symptoms like nausea,vomiting,photophobia,and phonophobia occur more often in migraine than in CGH.Moreover,migraine often affects the temporal area,whereas CGH is more likely to affect the occipital and temporal areas.Patients with migraine or CGH have similar clinical responses to the headache prescription that dispels pathogenic wind,eliminates dampness,removes blood stasis,and tonifies deficiency.
