利培酮治疗额颞脑挫裂伤后精神症状的临床研究

摘　　要：目的观察利培酮在额颞脑挫裂伤后精神症状临床治疗中的疗效。方法选取2009年1月至2011年12月期间我院收治的60例额颞脑挫裂伤后伴有精神症状患者作为观察对象,随机分为对照组和治疗组,对照组患者给予维生素B160mg/d,治疗组患者给予利培酮1mg/d,2组疗程均为1个月,分别于治疗前、后使用阳性与阴性症状量表(PANSS)与症状量表(TESS)对两组患者的治疗效果及不良反应进行评价,对结果进行对比分析。结果治疗组治疗开始后第1、2、3、4周的PANSS评分明显低于对照组,两组对比差异具有统计学意义(F=48.12,P<0.0001),时间对比差异具有统计学意义(F=290.99,P<0.0001),组间与时间交互作用具有统计学意义(F=11.91,P<0.0001)。经控制时间点后两组比较具体表现为治疗开始后第1、2、3、4周两组对比差异具有统计学意义。治疗过程中两组患者均出现不同程度的头痛、反胃、体重增加以及心动过速等不良反应,均经对症处理后明显缓解。结论利培酮可明显改善患者额颞脑挫裂伤后精神症状,安全有效。ObjectiveTo examine the effect of risperidone on the mental symptoms after frontotemporal brain contusion.MethodsSixty cases with mental symptoms after frontotemporal brain contusion were recruited from Jan 2009 to Dec 2011 and were randomly divided into control group and treatment group. The patients in the control group were given vitamin B1 60mg/d, while the patients in the treatment group were given risperidone 1mg/d. Both groups were treated for 1 month. Positive and negative symptom scale（PANSS） and symptom scale（TESS） were used to evaluate the efficacy and adverse reactions of treatment.ResultsThe PANSS score was significantly lower in the treatment group than in control group at 1, 2, 3 and 4 weeks following treatment（difference between groups： F=48.12,P〈0.0001; Time difference：F=290.99,P〈0.0001; Interaction between group and time： F=11.91,P〈0.0001）. After time-adjustment, the PANSS score was significantly lower in the treatment group than in control group at 2, 3 and 4 weeks following treatment. In the course of treatment, the patients in both groups had varying degrees of headache, nausea, weight gain and Beckoning.These side effects were alleviated through symptomatic treatment.ConclusionRisperidone can significantly improve psychiatric symptoms in patients with frontotemporal brain contusion with satisfactory safety.
