摘　　要：目的探讨以聚丙烯酰胺凝胶为血管栓塞剂对家兔行肝动脉栓塞的效果。方法选择15只家兔,通过右股动脉—肝动脉置入微导管并注入聚丙烯酰胺凝胶栓塞剂,造影观察肝动脉的栓塞情况。于术前1 d、术后1、3、7 d取外周耳缘静脉血检测ALT、AST水平变化。术后3、7、14 d各取2只解剖,取出肝脏,大体观察肝脏表面情况,并制备切片行HE染色,光学显微镜下观察栓塞后肝脏组织病理学改变。结果术中栓塞剂推注过程顺利,均无微导管堵塞情况;栓塞后复造影,肝细小动脉及其分支、末梢动脉被栓塞未显影。术后1、3 d ALT、AST均高于术前(P均<0.05)。术后3、7 d ALT、AST均低于术后1 d(P均<0.05)。栓塞后3 d肝周结缔组织炎症反应明显,肝血窦及汇管区炎细胞浸润,并见血栓形成。结论聚丙烯酰胺凝胶栓塞剂对家兔肝动脉栓塞效果确切,可彻底栓塞目标肝动脉,对肝功能损害小,是一种理想的血管栓塞材料。Objective To observe the effect of polyacrylamide （ PAM） gel as the embolic agent in rabbit hepatic arte-rial embolism.Methods Fifteen rabbits were selected , and PAM gel was injected into the right femoral artery-hepatic ar-tery via microcatheter .Hepatic artery embolization was observed by radiography .On preoperative 1 d and postoperative 1 d, 3 d and 7d, the changes of alanine aminotransferase （ALT） and aspartate aminotransferase （AST） levels in the periph-eral venous blood was detected .On postoperative 3, 7 and 14 d, we dissect two from each group , took out the liver to gen-erally observe the situation of liver surface and prepared sliced of HE staining to observed pathological changes after emboli -zation under an optical microscope .Results During the operation , the injection process of embolic agents was smooth , and no microcatheter jammed .After embolism, angiography showed that small hepatic artery and its branches , peripheral artery were undeveloped.On postoperative 1 and 3 d, ALT and AST were higher than that before operation （all P〈0.05）. On postoperative 3 and 7 d, ALT and AST were much lower than that on postoperative 1 d （all P〈0.05）.The inflamma-tion was evident at liver surrounding connective tissues , inflammatory cell infiltrated liver sinusoids and portal area with thrombosis after embolization .Conclusion PAM gel as an embolic agent for rabbit hepatic artery has exact effect .It can completely embolize objective liver tissues , and has a little damage to liver functions .Polyacrylamide gel is an ideal materi-al for blood clots .
