摘　　要：目的:探讨胃癌中Ki-67与RAB5A在胃黏液腺癌中的表达情况,从分子水平探讨Ki-67和RAB5A与胃黏液腺癌分化程度以及浸润转移的相关性,为胃黏液腺癌的早期诊断和治疗提供一种更加敏感和可靠的依据。方法:应用过氧化物酶标记链霉卵白素法(S-P法)来检测18例印戒细胞癌和42例胃黏液腺癌、22例正常胃黏膜组织中Ki-67和RAB5A蛋白的表达水平及之间的相关性。通过免疫组织化学检测并结合患者的性别年龄、肿瘤组织浸润分化程淋巴结转移以及腹膜种植等情况对胃黏液腺癌以及癌旁3.0cm组织和正常组织进行综合分析。结果:Ki-67和RAB5A免疫反应阳性细胞为棕黄色,多数存在于细胞质或细胞膜。按照临床病理分析结果表明:Ki-67和RAB5A的表达与侵润深度、淋巴结转移和腹膜种植有着密切的关系,而与性别、年龄、肿瘤大小无关(P>0.05)。侵及浆膜组Ki-67和RAB5A蛋白的阳性表达率分别为89.8%和95.9%,而未侵及浆膜组织Ki-67和RAB5A蛋白的阳性表达率分别为64.3%和57.1%,比较结果有显著性差异(P<0.01);淋巴结转移组的Ki-67与RAB5A蛋白的阳性表达率分别为87.9%和90.9%,而无淋巴结转移组的Ki-67与RAB5A蛋白的阳性率为66.7%和63.7%,两者比较结果差异显著(P<0.05);有腹膜种植的Ki-67和RAB5A蛋白的阳性分别为79.3%和86.2%差异较明显(P<0.05),无腹膜种植情况Ki-67和RAB5A蛋白的阳性分别为77.4%和70.9%,差异无显著性(P>0.05)。结论:Ki-67和RAB5A可能与胃黏液腺癌的发生和发展有密切的关系;胃黏液癌中存在Ki-67和RAB5A表达上调,临床上可以通过检测Ki-67和RAB5A提高胃粘液腺癌早期诊断和早期治疗的敏感性。Objective： To study the expression of Ki- 67 and RAB5 A in mucinous gastric carcinoma. The correlation among Ki- 67 and RAB5 A and lymph node metastasis in invasive depth and degree of differentiation infiltration of mucinous gastric carcinoma were tested from molecular level,which provides a more sensitive and reliable basis for the early diagnosis and treatment of mucinous gastric carcinoma. Methods： S- P immunohistochemical technique is used to examine the expression and relation of Ki- 67 and RAB5 A in 42 cases of mucous gastric carcinoma,18 cases of a signet- ring cell carcinoma and 22 cases of normal gastric tissues. The mucinous gastric carcinoma,3. 0cm adjacent tissue and normal tissues were comprehensively analyzed by immunohistochemical detection combined with gender,age,tumor size,lymph node metastasis in invasive depth,the differentiation of tumor infiltration,and distant metastasis of mucinous gastric carcinoma. Results： The color of the immune positive products of Ki- 67 and RAB5 A was buffy and mostly located in cytoplasm or plasmalemma. Clinical pathologic analysis results showed that the expressions of Ki- 67 and RAB5A had no significant differences in age,sex,and the size of tumor（ P 〈0. 05）. The lymph node metastasis in invasive depth and the degree of differentiation have significant differences between mucinous gastric carcinoma and signet- ring cell carcinoma. The rate of positive expression of Ki- 67 and RAB5A in serosa stratum was 90. 1% and 96. 7%. But the positive expressions out of serosa stratum were 64. 3% and 57. 1%. The expressions of Ki- 67 and RAB5 A were higher in gastric carcinoma with lymph node metastasis than those without lymph node metastasis. The rate of positive expression were 87. 9%,90. 9% and 66. 7%,63. 7% respectively（ P 〈0. 05）. The expression of Ki- 67 and RAB5 A were 79. 3% and86. 2% in gastric carcinoma with Peritoneal planting,and the rate of positive expression were 77. 4% and 70. 0%,respectively（ P 〈0. 05）.
