摘 要:目的比较围术期肠内营养(EN)与肠外营养(PN)支持用于食管癌合并糖尿病患者的临床疗效。方法选取2013年6月~2014年6月在内蒙古医科大学附属人民医院接受手术治疗的食管癌合并糖尿病患者50例,按照随机数字表法分为EN组和PN组,各25例;两组分别给予EN支持和PN支持,疗程为术前3 d~术后8 d。观察并对比两组围术期血糖控制情况、术前1 d及术后8 d的营养状况、术后胃肠功能恢复时间、营养支持相关并发症及营养支持花费。结果动态血糖监测结果显示,两组患者的血糖控制情况比较满意,组间比较差异无统计学意义(P〉0.05);两组患者的营养状况指标及营养支持相关并发症发生率比较,差异无统计学意义(P〉0.05);EN组和PN组患者术后胃肠功能恢复时间分别为(5.35±0.77)d和(7.82±0.93)d,EN组明显早于PN组,差异有统计学意义(P〈0.05);EN组和PN组患者营养支持花费分别为(720.46±65.32)元和(2894.35±394.05)元,EN组明显少于PN组,差异有高度统计学意义(P〈0.01)。结论围术期营养支持可以在有效控制血糖的同时明显改善食管癌合并糖尿病患者的营养状况,与PN支持相比EN支持能更快地恢复胃肠功能,并减轻患者的经济负担。Objective To compare the clinical curative effect of enteral nutrition(EN) and parenteral nutrition(PN)support for esophageal cancer patients with diabetes mellitus in perioperative period.Methods Selected 50 cases of esophageal cancer patients with diabetes mellitus from June 2013 to June 2014 in Affiliated People's Hospital of Inner Mongolia Medical University,divided into EN group and PN group according to random number table method,each group had 25 cases.Two groups were respectively given EN and PN support,course of the treatment between 3 d before surgery and 8 d after surgery.The perioperative control status of blood glucose,nutritional status at the time of 1 d before surgery and 8 d after surgery,time of gastrointestinal function recovery,related complications of the nutrition support and the cost of nutritional support of the two groups were observed and compared.Results The results of dynamic blood sugar monitoring showed that the control status of blood glucose of the two groups were satisfied,there was no statistically significant difference between the two groups(P〈0.05).The indicators of the nutritional status and the related complications of the nutritional support of patients in the two groups had no statistically significant differences(P〉0.05).Time of the gastrointestinal function recovery of the EN group and the PN group was(5.35±0.77) d and(7.82±0.93) d respectively,the EN group was obviously earlier than the PN group,the difference was statistically significant(P〈0.05).The cost of the nutrition support of the EN group and PN group was(720.46 ±65.32) yuan and(2894.35±394.05) yuan respectively,EN group was significantly less than the PN group,the difference was statistically significant(P〈0.01).Conclusion The nutrition support can be in effective control of blood sugar and improve the nutritional status of the esophageal cancer patients with diabetes mellitus in perioperative period,EN support can quicker recover the gastrointestinal functi
