强迫症患者儿童期被忽视及追求完美与强迫症严重程度的相关性研究

摘　　要：目的探讨强迫症患者儿童期被忽视、追求完美与强迫症严重程度的关系以及儿童期被忽视的中介作用。方法选取2011年12月—2012年11月在新乡医学院第二附属医院治疗的强迫症患者76例,采用耶鲁-布朗强迫症状量表(Y-BOCS)、儿童期被忽视量表(CNS)、中文Frost多维度完美主义问卷(FMPS)进行评估,采用层次回归法和依次检验回归系数法评估和检验儿童期被忽视在追求完美与强迫症严重程度之间的中介作用。结果Y-BOCS评分为(19.4±2.6)分、CNS评分为(104.0±6.6)分、FMPS评分为(111.3±4.6)分。Y-BOCS评分与CNS评分呈正相关(r=0.409,P<0.001);Y-BOCS评分与FMPS评分呈正相关(r=0.364,P=0.001);CNS评分与FMPS评分呈正相关(r=0.295,P=0.010)。层次回归分析结果显示,追求完美通过儿童期被忽视的中介作用对强迫症严重程度产生间接影响,中介效应值约为:0.097/0.267=36.3%。结论儿童期被忽视的程度越高,强迫症程度越严重。追求完美除对强迫症严重程度有直接影响外,还通过儿童期被忽视对强迫症严重程度产生间接影响。Objective To explore the correlation among childhood neglect,pursuing perfection and the severity of obsessive- compulsive disorder,and to explore the mediating effect of childhood neglect. Methods 76 patients with obsessive-compulsive disorder who were treated in the Second Affiliated Hospital of Xinxiang Medical University from December 2011 to November 2012,were selected as study subjects and investigated by Yale- Brown Obsessive Compulsive Scale（ Y- BOCS）,Childhood Neglect Scale（ CNS） and Chinese Frost Multidimensional Perfectionism Scale（ FMPS）. Hierarchical regression and the regression coefficient method（ successive steps） were used to test the mediating effect of childhood neglect between pursuing perfection and the severity of obsessive- compulsive disorder. Results The score of Y- BOCS was（ 19. 4 ± 2. 6）,the score of CNS was（ 104. 0 ± 6. 6）,the score of FMPS was（ 111. 3 ± 4. 6）. The score of Y- BOCS was positively correlated with the score of CNS（ r = 0. 409, P〈0. 001）; the score of Y- BOCS was positively correlated with the score of FMPS（ r = 0. 364,P = 0. 001）; the score of CNS was positively correlated with the score of FMPS（ r = 0. 295, P = 0. 010）. The results of hierarchical regression indicated that pursuing perfection had the indirect effect on the severity of obsessive- compulsive disorder by the mediating effect of childhood neglect,and the intermediary effect was about 36. 3%（ 0. 097 /0. 267）. Conclusion The higher degree of childhood being neglected,the more serious is the severity of obsessive- compulsive disorder. Pursuing perfection has not only a direct impact on the severity of obsessive- compulsive disorder,but also has an indirect effect on the severity of obsessive- compulsive disorder through the childhood neglect.
