超声导向下经直肠穿刺I^(125)植入治疗前列腺癌的临床疗效

摘　　要：目的探讨对前列腺癌患者在超声导向下经直肠穿刺I125植入治疗的临床疗效及安全性,为临床上前列腺癌的治疗提供参考建议。方法随机将作者医院收治的120例前列腺癌患者分成观察组和对照组各60例,对照组采用根治切除术进行治疗,观察组采用经直肠超声导向经直肠穿刺I125植入治疗。比较两组患者的手术时间、住院时间、PSA(术前、术后3个月、6个月、9个月)、最大尿流率、国际前列腺症状评分(international prostate symptom score,IPSS)及尿路和直肠并发症的发生情况。结果观察组患者平均植入穿刺针14根,植入粒子平均(42±17)粒。观察组在手术时间、住院时间上均显著的短于对照组患者(P<0.05)。对照组所有患者术后4~7 d拔出尿管后均能够正常排尿,观察组患者有2例出现拔出尿管后不能自行排尿,经过对症处理后缓解。术后对照组有7例患者出现尿频、尿急、尿痛等尿路刺激症状,观察组有9例出现上诉症状,随访至6个月后,两组患者的上诉症状均显著的好转。结论超声导向下经直肠穿刺I125植入治疗前列腺癌是一种微创、安全、有效的治疗方法,可以显著缩短手术及住院时间,同时不会增加术后并发症的发生率。Objective To explore the clinical curative effect and safety of patients with prostate cancer in ultrasound guided transrectal biopsy of I125 implantation,providing reference for clinical treatment of prostate cancer.Methods 120 cases of prostate cancer patients in our hospital were divided into observation group and control group with 60 cases in each group.The control group were treated by radical resection,and the observation group was treated with ultrasound guided transrectal puncture I125 implantation.Compared the operation time,hospitalization time,two groups of patients with PSA（preoperative,postoperative 3 month,6 months,9 months）,maximum urine flow rate,International Prostate Symptom Score（IPSS）and urinary rectal complications.Results The patients in observation group average needle implantation 14,average particle implantation（42±17）grain.The operation time and hospitalization time of the observation group were significantly shorter than those of the control group（P〈0.05）.All patients in the control group after operation 4~7 d pulled out the catheter were able to urinate normally,2 of the patients in observation group pulled out the catheter were unable to urinate,and remission was got after symptomatic treatment.After operation there were 7 patients in the control group facing micturition,urgent urination,odynuria urinary symptoms,and 9 cases in the observation group had above symptoms.The symptoms of the two groups significantly improved after follwing up for 6 months.ConclusionUltrasound guided transrectal puncture I125 implantation in the treatment of prostate cancer is a minimally invasive,safe and effective therapy,which can significantly shorten the operation and hospitalization time and does not increase the incidence of postoperative complications.
