围绝经期及绝经后妇女附件包块126例临床病例分析

摘　　要：目的：探讨分析围绝经期及绝经后妇女附件包块的早期诊断及临床处理。方法：回顾性分析2012年1月~2013年9月在我院收治的126例围绝经期及绝经后妇女附件包块患者的临床病例资料。结果：126例围绝经期及绝经后妇女附件包块36例患者无任何症状,占28.6%,其性质多为良性,其中卵巢良性肿瘤与恶性肿瘤的比例为1.55：1,直径〉9cm的包块的恶性率高于直径〈9cm的包块,囊实性包块以及实性包块的恶性率高于包块,双侧、多房囊性、伴有腹水、伴有血清CA125升高的包块的恶性率高于单侧、单房囊性、无腹水、CA125正常的包块。结论：每年给予围绝经期及绝经后妇女妇科普查非常重要,阴道彩超及血清CA125检测可初步判断围绝经期及绝经后妇女附件包块的良恶性,对于包块直径〉9cm、双侧、多房囊性、伴有腹水、伴有血清CA125升高的包块恶性可能性大,临床应慎重处理,选择合适的处理方案。Objective：To study the analysis of perimenopausal and postmenopausal women accessory bag piece of early diagnosis and clinical treatment.Methods：126 patients of our hospital were retrospectively analyzed clinical data.Results：126 cases with 36 patients without any symptoms, accounted for 28.6%,whose nature is benign,more percentage of ovarian benign tumor and malignant tumor for 1.Thyself,bag piece 〉9cm in diameter malign rate is higher than the diameter of 〈9cm bag piece,pouch or solid mass and malignant solid mass rate is higher than that of bag piece,double side,multilocular cystic,accompanied by ascites,associated with elevated serum CA125 of bag piece of cystic malignant rate is higher than single,single room, no ascites and CA125 normal bag piece.Conclusion：Vaginal colour to exceed and serum CA125 detection can preliminary judgment perimenopausal and postmenopausal women of benign and malignant accessory bag piece, for bag piece 〉9cm in diameter,double side,multilocular cystic,accompanied by ascites,associated with elevated serum CA125 of malignant possibility big bag piece.
