老年人群缺血性脑卒中复发风险的预测

摘　　要：目的:探讨颈动脉彩色多普勒超声(CDFI)检查联合高同型半胱氨酸(Hcy)血浆水平预测老年人缺血性脑卒中(CIS)患者复发的风险。方法:回顾性分析我院急救部救治的613例脑梗塞患者的临床资料,其中符合诊断标准96例,复发次数≥1次;记录患者入院时CDFI、荧光偏振免疫法检测Hcy血浆水平变化;分别进行复发组患者与未复发组患者颈动脉CDFI及高Hcy血浆水平的分析;应用ROC曲线判断CDFI和高Hcy血浆检测水平,以判断二者对CIS复发风险的预测价值。结果:颈动脉斑块或狭窄在复发组患者中的比率高于未复发组患者(P<0.05);复发患者血浆Hcy水平高于未复发患者(P<0.05)。高血浆Hcy水平并颈动脉溃疡型混合斑块或狭窄患者多的患者复发率最高(P<0.05);结论:CDFI和血浆Hcy水平均对CIS复发风险有预测价值,二者联合的预测价值更好。Objective： To investigate the carotid artery color Doppler ultrasound（CDFI） examination combined with high homocysteine（Hcy） plasma levels predict Cerebral ischemic stroke（CIS） in the elderly risk of recurrence in patients. Methods： Retrospectively analyzed the clinical data of 613 cases of cerebral infarction patients in Department of emergency treatment, which conforms to the diagnosis standard of 96 cases of recurrence, with more than 1 times; CDFI, record the changes of fluorescence polarization immunoassay for detection of Hcy plasma levels in patients hospitalized patients; analysis of recurrence of patients with non recurrent group of carotid artery in patients with high plasma levels of CDFI and Hcy separately; application of ROC curve to determine the detection of plasma level of CDFI and high Hcy, to determine the two on the risk of CIS recurrence prediction value. Results： The rate of carotid artery plaque or stenosis in recurrent group was higher than in patients with recurrent patients（P〈0.05）; plasma Hcy levels in patients with relapse than that of non recurrence patients（P〈0.05）. The high plasma Hcy levels and carotid ulcer type mixed plaque or stenosis in patients with multiple patients relapse rate was highest（P〈0.05）. Conclusion： CDFI and plasma Hcy levels are predictive value for the risk of CIS recurrence, the predictive value of the combination of the two in the better.
