摘　　要：十二指肠残端瘘是腹腔镜胃癌根治术后最为严重的并发症之一,有较高的发生率和病死率。十二指肠残端瘘的早期诊断对患者的预后非常重要,对于存在高危因素的患者手术后需要严密观察。十二指肠残端瘘一经诊断明确,需要结合具体情况,如患者一般状态、是否局限、十二指肠残端引流情况等,决定保守治疗或是手术治疗,其基本原则是保证十二指肠残端的通畅引流。发生十二指肠残端瘘后及时诊断并给予正确的处理才能避免因此导致的严重后果。Duodenal stump leakage is one of the most serious complications after laparoscopic gastrectomy with high mortality and morbidity. Early diagnosis is particularly important for a good prognosis and more attention should be paid to patients with high risk after surgery. Once duodenal stump leakage occurred after laparoscopic radical gastrectomy, proper treatment should be done, including conservative treatment and/or reoperation based on patients＇general conditions, the extent of leakage and drainage status, and the guiding principle of fluent drainage. Prompt diagnosis and appropriate treatment could help to avoid the serious consequences of the duodenal stump leakage after laparoscopic gastrectomy.
