T细胞亚群手术前后变化与胃肠癌根治术后感染性并发症的关系

摘　　要：目的探讨胃肠癌根治术后感染并发症与T细胞亚群手术前后变化的关系。方法选择60例胃肠道恶性肿瘤根治术后感染患者为观察组,60例胃肠道恶性肿瘤术后未合并感染的患者为对照组,检测并比较术前、术后1、4和9 d检测两组患者的CD3、CD4、CD8和CD4/CD8水平。结果术前,两组患者的CD3、CD4、CD8和CD4/CD8间比较差异无统计学意义(P>0.05)。观察组患者的CD3、CD4和CD4/CD8于术后1、4 d呈下降趋势,CD8呈上升趋势,术后9 d恢复至术前水平。对照组患者的CD3、CD4和CD4/CD8于术后1 d明显下降,术后4 d回升,术后9 d恢复至术前水平。CD8于术后1 d上升,术后4 d开始下降,术后9 d恢复至术前水平。两组患者的CD3、CD4、CD8和CD4/CD8的差异均有统计学意义(P<0.05)。且术后4 d,观察组患者的CD3、CD4和CD4/CD8显著低于对照组,CD8显著高于对照组。结论胃肠癌根治术后感染患者的CD3、CD4、CD8和CD4/CD8水平存在一定特征,可用于患者病情监测。Objective To analyze the relationship between T cell subsets variation curves before and after gastric cancer surgery and radical resection of infectious complications. Methods A total of 60 cases of patients with infection after malignant gastrointestinal tumors were chosen as the research objects, 60 malignant gastrointestinal tumors without infection after surgery as control group. CD3, CIM, CD8 and CI）4/CD8 of two groups in preoperative and 1, 4, 9 d postoperative were compared. Results Before the operation, there were no significant differences between CD3, CD4, CD8 and CD4/CD8 of two groups （ P 〉 0.05）. In the observation group, CD3, CD4 and CD4/CD8 after operation in 1, 4 d decreased, CD8 in- creased, recovered to the preoperative level in postoperative 9 d. In the control group, CD3, CD4 and CD4/CD8 decreased significantly in 1 d after operation, recovered to the preoperative level in postoperative 9 d. CD8 raised in 1 d postoperative, decreased in 4 d postoperative, recovered to the preoperative level in postoperative 9 d. There were obvious differences of CD3, CD4, CD8 and CD4/CD8 curves in two groups （ P 〈 0.05 ）. And 4 d after operation, CD3, CD4 and CD4/CD8 of the observation group were significantly lower than those of control group, CD8 was significantly higher than that of control group. Conclusion There are certain characteristics of patients with CD3, CD4, CD8 and CD4/CD8 levels after radical resec- tion of gastric cancer infection, and could be used for patient monitoring.
