经直肠超声造影定量分析对前列腺癌的鉴别与诊断价值探讨

摘　　要：目的探讨经直肠超声造影定量分析对前列腺癌(COP)的诊断价值。方法回顾性分析新疆医科大学第五附属医院泌尿外科2010年9月至2014年1月间收治的105例COP患者的临床资料,病灶均位于前列腺外腺,经超声造影后绘制时间-强度曲线,获取超声造影相关定量参数,如达峰时间(TP)、曲线下面积(AUC)、峰值强度(PK)及斜率。结果 105个COP病灶及其周围未受累组织的AUC、斜率比较,差异有统计学意义(t=11.863、3.574,P<0.05),而两组TP和PK比较,差异无统计学意义(t=1.446、1.055,P>0.05)。COP周围未受累组织与内腺增生组织的TP、AUC、PK及斜率比较,AUC、PK差异有统计学意义(t=-9.362、2.010,P<0.05),TP和斜率比较差异无统计学意义(t=0.336、0.515,P>0.05)。结论经直肠超声造影定量分析对COP具有鉴别价值,能为COP定性诊断提供更多可靠信息。Objective To investigate the diagnosis value of contrast-enhanced transrectal ultra- sonography for carcinoma of prostate. Methods 105 cases of carcinoma of prostate （COP） clinical data were retrospective analyzed from September 2010 to January 2014. All the lesions were located outside the prostate gland. After ultrasound contrast, time - intensity curve （TIC） was made, and relevant quantitative parameters such as ultrasound contrast peak time （ TP ）, area under the curve （ AUC ）, peak intensity （PK） and slope were obtained. Results The difference of AUC and the slope between 105 eases of COP lesion and unaffected surrounding tissue was statistically significant （ t = 11. 863, 3.574, P 〈 0. 05 ）, while TP, PK of the two group had no significant difference （ t = 1. 446, 1. 055, P 〉 0. 05 ）. The difference of AUC and PK between unaffected surrounding tissue and the inner gland hyperplasia was statistically significant （ t = -9. 362, 2. 010, P 〈 0. 05 ）, and there was no significant difference in TP and the slope （ t = 0. 336, O. 515, P 〉 0. 05）. Conclusion It＇s valuable of quantitative analysis of contrast-enhanced transrectal ultra- sonography for carcinoma of prostate, can provide more reliable information for the COP diagnosis.
