阿片类药物联合超声药物离子导入治疗癌痛对晚期癌症患者生存的影响

摘　　要：目的探讨阿片类药物联合超声药物离子导入治疗癌痛对晚期癌症患者生存的影响。方法选取本院2013年10月~2014年10月收治入院的中重度癌痛患者（均为病理组织学或细胞学证实为恶性肿瘤）80例,按随机数字表法分为治疗组和对照组各40例,两组患者近期未进行放化疗治疗,对照组按WHO推广的癌症＂三阶梯止痛＂原则给药,治疗组在此基础上给予阿片类药物联合超声药物离子导入治疗,采用视觉模拟评分（VAS）法评估疼痛程度,比较两组的临床疗效、生活质量。结果治疗组总有效率为90.0%,高于对照组的70.0%,差异有统计学意义（χ^2=4.126,P〈0.05）;治疗组生活质量评分（QOLC-E）量表中身体状况、认知功能、情绪状况、社会功能、行为问题、自评健康、自评生活质量、总评分均明显高于对照组,差异有统计学意义（P〈0.05）。结论阿片类药物联合超声药物离子导入治疗可显著提高癌痛患者的生活质量。Objective To investigate the effect of opioid drugs combined with ultrasound drug iontophoresis treatment of cancer pain on the survival of patients with advanced cancer. Methods 80 cases of patients with moderate to severe cancer pain（histologically or cytologically confirmed malignant tumor） treated in our hospital from October 2013 to October 2014 were selected,and were randomly divided into treatment group and control group,40 cases in each group,were not given with chemoradiotherapy treatment recently.Control group were given with ＂three steps analgesic＂ principle which the WHO promoted,opioid drugs combined with ultrasound drug iontophoresis treatment was taken to treatment group on the basis of control group.Visual analog score（VAS） was used to evaluate the degree of pain,the clinical curative effect and quality of life between the two groups was compared. Results The total effective rate of treatment group was 90.0%,higher than 70.0% of control group,the difference was significant（χ^2=4.126,P〈0.05）.QOLC-E scale scores of physical status,cognitive function,social function,emotional status,behavioral problems,self-rated health,quality of life,the total score of self rating in treatment group was higher than that in control group,the difference was significant（P〈0.05）. Conclusion Opioid drugs combined with ultrasound drug iontophoresis treatment of cancer pain in patients with advanced cancer can significantly improve the quality of life of patients with cancer pain.
