立体定向放疗联合肝动脉化疗栓塞对结直肠癌肝转移治疗的研究

摘　　要：目的探讨立体定向放疗联合肝动脉化疗栓塞治疗结直肠癌肝脏转移瘤的临床应用价值。方法选取100例结直肠癌肝转移患者为研究对象,根据随机数字表法将患者分为两组,每组50例。观察组采用立体定向放疗联合肝动脉化疗栓塞治疗,对照组采用单纯肝动脉化疗栓塞治疗。比较两组患者临床疗效、生活治疗评价及不良反应。结果观察组有效率显著高于对照组(P<0.05);观察组1年生存率和2年生存率均显著高于对照组,差异有统计学意义(P<0.05)。观察组生理状况、家庭/社会状况、情感状况、功能状况及量表总分均显著高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率间比较差异无统计学意义(P>0.05)。结论立体定向放疗联合肝动脉化疗栓塞治疗结直肠癌肝脏转移瘤疗效优于单纯肝动脉化疗栓塞,并可以提高患者生活质量。Objective To investigate the clinical application value of stereotactic radiotherapy combined with transcatheter arterial chemoembolization for the treatment of colorectal liver metastases. Methods 100 patients with liver metastasis in colorectal cancer patients were included as the research object. The patients were divided into observation group and control group according to the random number table method. The observation group were treated with stereotactic radiotherapy combined with transcatheter arterial chemoembolization therapy. The control group used a simple treatment of hepatic arterial chemoembolization. Curative effect evaluation of treatment of life of the patients,and adverse reaction of two groups were compared. Results CR and PR ratio in the observation group was significantly higher than that of control group（ P〈0. 05）.The observation group,1 years survival rate and the 2 year survival rate were higher than the control group. There was significant difference（ P〈0. 05）. The observation group physiological status,family / social status,affective status. Functional status and the total scores were significantly higher than those of control group（ P〈0. 05）. The incidence of adverse reaction between two groups have no significant difference（ P〉0. 05）.Conclusion The stereotactic radiotherapy combined with transcatheter arterial chemoembolization for the treatment of colorectal liver metastases clear curative effect,and can improve the life quality of the patients.
