立体定向伏隔核毁损术治疗酒精依赖患者术后健康状况及生存质量调查

摘　　要：目的调查立体定向伏隔核毁损术治疗酒精依赖患者术后健康状况及生存质量。方法选择在第四军医大学唐都医院行立体定向伏隔核毁损术治疗酒精依赖患者31例为研究对象,术后1年对患者进行随访调查,了解患者术后的健康状况以及生存质量,分析该手术方法的安全性和有效性。结果 1年内戒断率90.3%(28/31)。治疗后自我感觉健康状况较术前良好19例,占61.3%,感觉较术前好了一些10例,占32.3%,觉得和术前差不多2例,占6.5%,无自我感觉健康状况较术前差的患者。健康状况问卷调查结果显示患者术后12个月生理机能、生理职能、躯体疼痛、一般健康状况、情感职能、精神健康等各领域得分均较术前显著升高(P<0.01)。世界卫生组织生存质量测定量表简表调查结果显示术后患者生理领域、心理领域、社会关系领域、一般健康状况、生存质量领域得分及自我生存质量评分均较术前显著升高(P<0.01)。结论立体定向伏隔核毁损术治疗酒精依赖具有较好的临床疗效,能够显著改善患者的健康状况以及生存质量。Objective To investigate postoperative health conditions and life quality for patients with alcohol depen- dence treated by stereotactic lesioning of nucleus accumbens. Methods All 31 patients with alcohol dependence who received stereotactic lesioning of nucleus accumbens in Tangdu Hospital of the Fourth Military Medical University were selected as research subjects. Follow-up investigation was carried out for patients one year after the surgery. Patients＂ postoperative health conditions and life quality were understood, so as to analyze the safety and effectiveness of the surgical procedure. Results The abstinence rate within one year was 90.3%（28/31）. 19 patients felt that their health conditions after the surgery were much better than those before the surgery, accounting for 61.3% of the total patients. 10 patients felt that their health conditions after the surgery were somewhat better than those before the surgery, accounting for 32.3% of the total. 2 patients felt that their health conditions were the same as those before the surgery, accounting for 6.5% of the total. No patients felt that their health conditions were worse than those before the surgery. Results of the questionnaire on health conditions showed that scores of physiological function, role physical, physical pain, general health condition, role emotional, mental health and other domains within 12 months after the surgery were all significantly higher than those before the surgery （P〈0.01）. Results of WHO Quality of Life-BREF showed that scores of physical domain, psychological domain, social relation domain, general health condition and life quality do main after the surgery were all significantly higher than those before the surgery （P〈O.O1）. Conclusion Stereotactic lesion- ing of nucleus accumbens in the treatment of alcohol dependence has a favorable clinical effect, and is capable of sig- nificantly improving patients＂ health conditions and life quality.
