摘 要:目的探讨诱导型一氧化氮合酶(inducible nitric oxide synthase,iNOS)在鼻咽癌中的表达与鼻咽癌生物学行为的关系,探讨肿瘤血管生成与鼻咽癌生物学行为的关系,探讨iNOS的表达与鼻咽癌血管生成的关系。方法采用非生物素二步免疫组化法,检测iNOS在鼻咽癌组织中表达,并根据CD34相关抗原标记血管内皮细胞。结果鼻咽癌组织中MVD显著高于慢性鼻咽炎组织(27.7±8.88 vs 9±3.1,P<0.05)。MVD与年龄、性别无关,临床分期Ⅲ、Ⅳ期组的MVD显著高于Ⅰ、Ⅱ期组(30.7±4.7 vs 22.2±7.5,P<0.05)。淋巴转移组MVD显著高于淋巴结未转移组(29.6±4.8 vs 20.5±4.8,P<0.05)。iNOS在鼻咽癌组织中阳性表达率为56.25%,iNOS的表达与年龄、性别无关。临床分期Ⅲ、Ⅳ期组iNOS的阳性表达率为70.97%,显著高于Ⅰ、Ⅱ期组29.41%(P<0.05)。淋巴转移组iNOS的阳性表达率为65.79%,显著高于淋巴结未转移组20.00%(P<0.05)。iNOS阳性表达组的MVD显著高于iNOS阴性表达组(30.0±5.4 vs 24.0±6.0,P<0.05)。结论血管生成及iNOS的表达与鼻咽癌生物学行为关系密切。iNOS表达促进了鼻咽癌血管生成及颈淋巴结的转移。测定MVD及iNOS的表达有助于判断鼻咽癌的恶性程度及预后。Objective:To explore the relationship between angiogenesis and biological behavior of nasopharyngeal carci-noma( NPC),the relationship between the expression of inducible nitric oxide synthase( iNOS)and biological behavior of NPC and the relationship between the expression of iNOS and angiogenesis of NPC. Methods:The expression of iNOS and micro vessel density( MVD)were assessed by immunohisto ± chemical methods in 48 specimens of NPC and in 13 speci-mens of chronic nasopharyngitis. The immuno-staining results and related chinical- pathological materials were analyzed by statistical methods. Results:MVD in NPC was significantly higher than that in chronic nasopharyngitis tissues(27. 7 ± 8. 88 vs 9 ± 3. 1,P〈0. 05). MVD in NPC was not related to such factors as age and gender. MVD ofⅢandⅣstages was higher than that ofⅠandⅡstages(30. 7 ± 4. 7 vs 22. 2 ± 7. 5,P〈0. 05). MVD of cases with lymphatic metastasis was sig-nificantly higher than that without lymphatic metastasis(29. 6 ± 4. 8 vs 20. 5 ± 4. 8,P〈0. 05). The positive staining rate of iNOS was 56. 25%. The expression iNOS was not related to age and gender. The positive expression rate(70. 97%)of iN-OS in cases ofⅢ andⅣ stages was higher than that(29. 41%)in cases ofⅠandⅡstages(P〈0. 05). The positive ex-pression rate(65. 79%)of iNOS in cases with lymphatic metastasis was higher than that(20. 00%)without lymphatic me-tastasis(P〈0. 05). MVD in the cases with positive expression of iNOS was higher than in the cases with in the cases with negative expression(30 ± 5. 4 vs 24 ± 6. 0,P〈0. 05). Conclusion:Angiogenesis of NPC is significantly related to the bio-logical behavior of NPC. The expression of iNOS is related to the biological behavior of NPC. The detection of MVD and the expression of iNOS in NPC can be used as indexes to determine the degree of malignancy and prognosis.
