5年复明工程小切口白内障摘除术后低视力原因分析

摘　　要：目的探讨5年来复明工程中小切口白内障囊外摘除联合人工晶体植入术的临床疗效及术后低视力的原因。方法分析2009年10月-2013年12月在我院"贫困白内障患者复明工程"中治疗的420例(420眼)患者的临床资料,观察随访末视力恢复情况,并分析低视力的原因。结果 383例(383眼)最佳矫正视力≥0.3,脱残率达91.2%;低视力者37例,分别是:糖尿病视网膜病变10例,老年性黄斑变性7例,后发性白内障7例,高度近视5例,视神经萎缩3例,特发性黄斑裂孔2例,视网膜中央动脉阻塞2例,眼内炎1例。结论小切口白内障囊外摘除联合人工晶体植入术应用在贫困白内障患者复明工程中安全、经济、有效;术后低视力主要是由术前眼底病变引起,应在术前谈话中重点与患者沟通。Objective：To investigate the clinical effects and the causes of low vision in patients who accepted small in-cision extracapsular cataract extraction combined with intraocular lens implantation in sight-recovery project. Methods：The clinical data of 420 cases（420 eyes）from October 2009 to December 2013 in our hospital were studied. We observed their postoperative visual function,and analyzed the causes of low vision. Results：The visual acuity of 383 cases（383 eyes） was over 0. 3. The removal rate of disability was 91. 2%. The causes of low vision among 37 patients were exhibited as be-low：diabetic retinopathy in 10 cases,senile macular degeneration in 7 cases,posterior capsular opacification in 7 cases, high myopia in 5cases,optic atrophy in 3 cases and 5cases of other causes. Conclusion：The small incision extracapsular cataract extraction combined with intraocular lens implantation in the sight-recovery project is safe,economic and effective. Low vision after the operation is mainly caused by preoperative fundus diseases to which more attention should be paid be-fore operation.
