摘 要:目的:观察白内障合并糖尿病患者行超声乳化术后角膜的变化情况。方法选取2012年1月~2014年10月在上海市徐汇区中心医院行白内障超声乳化术的老年白内障患者167例(207眼),根据有无合并糖尿病分为A、B两组,A组为单纯老年性白内障患者97例(119眼),B组为老年白内障合并糖尿病患者70例(88眼)。检测分析两组患者术前及术后3 d、1个月、3个月中央角膜厚度(CCT)、内皮细胞密度(CD)与内皮细胞平均面积(AVG)等情况。结果与术前比较,两组患者术后3 d中央角膜均明显增厚(P〈0.05);A组患者术后各时间点CD明显降低、AVG明显增加,差异均有统计学意义(P〈0.05);B组患者术后1、3个月CD明显降低、AVG明显增加,差异均有统计学意义(P〈0.05)。与A组患者比较,B组患者术后3个月CD明显降低、AVG明显增加,差异有统计学意义(P〈0.05)。结论相对于单纯老年性白内障患者,白内障合并糖尿病患者超声乳化术后角膜内皮细胞损伤更严重,持续时间与修复时间更长。Objective To observe the changes of cornea after ultrasonic phacoemulsification in patients with cataract combined with diabetes. Methods From January 2012 to October 2014, in Central Hospital of Xuhui District, 167 patients (207 eyes) received ultrasonic phacoemulsification were selected and divided into two groups, according to with diabetes or not, simple elderly cataract group (group A, 97 cases, 119 eyes), cataract combined with diabetes group (group B, 70 cases, 88 eyes). The values of central corneal thickness (CCT), corneal endothelial cell density (CD) and average area of corneal endothelial cells (AVG)were measured preoperatively as well as respectively in 3 d, 1 month and 3 months postoperatively. Results Compared with preoperative conditions, CCT in 3 days postoperatively in both groups significantly increased (P〈 0.05); CD of group A greatly reduced and AVG increased at various stages postoperatively, the differences were statistically significant (P〈0.05). While in 30 days and 90 days postoperatively, CD of group B significantly reduced, and AVG significantly increased, the differences were statistically significant (P〈0.05). Compared with group A, CD reduced obviously and AVG increased obviously in group B in 90 days postoperatively, the differences were statistically significant (P〈0.05). Conclusion Compared with simple elderly cataract, the damage of corneal endothelial cell after ultrasonic phacoemulsification in cataract combined with diabetes is severer and lasting longer, requiring more recovery time.
