摘　　要：目的：探讨肺淋巴管肌瘤病CT表现特征,以提高对该病的认识和诊断准确率。材料与方法：搜集我院8例经胸腔镜或穿刺活检病理证实为肺淋巴管患者的完整病例资料,均行高分辨CT（High-resolution CT,HRCT）检查及常规腹部CT检查。结合国内外文献,对该病的临床及CT征象进行分析。结果：8例均为女性,年龄22~49岁（平均年龄37岁）,就诊时均有不同程度进行性加重的劳力性呼吸困难,在常规扫描上见两肺散在或广泛均匀分布的小透亮影及囊状影,囊壁较模糊。HRCT上表现为均匀清晰薄壁小囊腔,囊腔直径几毫米至2厘米,壁厚1~2毫米,囊气腔被肺组织环绕,病变大小不一,分布无规律性;腹部CT示1例患者合并双肾血管平滑肌脂肪瘤,1例合并腹膜后及盆腔淋巴结肿大。结论：肺淋巴管肌瘤病以细支气管周围血管未成熟的平滑肌样细胞异常增生为特征,HRCT具有较特征性的表现,是本病诊断及鉴别诊断必不可少的影像检查方法。Purpose ： To study the CT findings of the pulmonary lymphangiomyomatosis for the improvement of recog- nition of this disease and precision rate of diagnosis. Materials and Methods： eight pulmonary lymphangiomyomatosis patients with pathological results by thoracoseopes or needle biopsy were all received High-resolution CT（HRCT） and routine abdominal CT examination. Clinical data and CT signs were analyzed and summarized combining with the papers both at home and abroad. Results： 8 patients all were female with the age range from 22-49 years old. They all presented with progressive exertional dyspnea in different degree. Diffuse distributed translucent and cystic leisions with fuzzy walls in bilateral lungs were showed on routine CT films. Cysts with diameter range from several millimeters to 2 centimeters and thin wall of thickness range from 1-2 millimeters were surrounded by the normal lung tissue on HRCT. The cysts were with different sizes and irregu- lar distribution. Bilateral renal angioleiomyolipomas were found in one patient and retroperitoneal and pelvic lymphadenectasis in another patient on abdominal CT films. Conclusion： Immature smooth muscle like cells with abnormal hyperplasia in the bronchioles peripheral vascular was the character of the pulmonary lymphangiomyomatosis. The pulmonary lymphangiomyoma- tosis presented with characteristic feature on HRCT. HRCT was the essential imaging method for the diagnosis and differential diagnosis of the pulmonary lymphangiomyomatosis
