摘 要:目的研究骨桥蛋白(osteopontin,OPN)及表皮生长因子受体(epidermal growth factor receptor,EGFR)、磷脂酰肌醇3-激酶(phosphatidylinositol-3-kinase,PI3K)蛋白在食管鳞癌中的表达及与其临床意义。方法应用S-P免疫组化技术检测70例食管鳞癌及其相应的癌旁正常组织中OPN和EGFR、PI3K蛋白的表达。结果 70例食管鳞癌组织中OPN、EGFR、PI3K蛋白的阳性表达率72.9%(51/70)、67.1%(47/70)、81.4%(57/70)均高于癌旁正常组织5.7%(4/70)、18.6%(13/70)、15.7%(11/70),差异具统有显著性(P<0.05);OPN的表达与淋巴结转移、临床分期相关,差异有显著性(P<0.05);EGFR蛋白的表达与浸润深度、淋巴结转移、临床分期相关差异有显著性(P<0.05);PI3K蛋白的表达与浸润深度、淋巴结转移、临床分期、分化程度相关差异有显著性(P<0.05)。OPN与EGFR、PI3K蛋白在食管鳞癌组织中的表达呈正相关关系(rs=0.325、0.287,P<0.05),EGFR与PI3K蛋白在食管鳞癌组织中的表达呈正相关关系(rs=0.292,P<0.05)。结论 OPN可能通过激活EGFR、PI3K、Akt信号系统在肿瘤的发生发展中发挥重要作用。Objective To investigate the relationship and clinical significance between the expressions of OPN and EGFR, PI3K proteins in esophageal squamous cell carcinoma. Method S-P Immunohistochemistry was used to de-tect the expression of OPN and EGFR, PI3Kproteins in pathological tissue with 70 cases with esophageal squamous cell carcinoma and tumor-adjacent normal esophageal tissue. Result The positive expression rates of OPN and EG-FR, PI3K proteins in 70 cases with esophageal squamous cell carcinoma tissue72. 9%(51/70),67. 1%(47/70), 81. 4%(57/70)respectively,were significantly different from the positive expression rates of tumor-adjacent normal esophagealtissues5.7%(4/70),18.6%(13/70),15.7%(11/70)respectively(P〈0.05);The expression of OPN was significantly correlated to TNM stage, ymph node metastasis(P〈0. 05)and the expression of EGFR proteins was significantly correlated to differentiation degree, TNM stage and lymph node metastasis(P〈0. 05). The expression of PI3K protein was significantly correlated to differentiation degree, TNM stage, tumor infiltrative depth and lymph node metastasis(P〈0. 05). There was positive correlation between the expression of OPN/ EGFR,OPN/PI3K and EGFR/PI3K proteins in esophageal squamous cell carcinoma and the value of Spearman coefficient correlation were 0. 325,0. 287 and 0. 292 (P〈0. 05). Conclusion OPN may play an important role in tumorigenesis and tumor pro-gression by activating EGFR/PI3K/Akt signaling pathway.
