摘 要:目的研究卒中后抑郁患者(post-stroke depression,PSD)抑郁情绪与Apo E基因多态性之间的相关性。方法采用24项汉密尔顿抑郁量表(HAMD-24)将病例分为PSD组(83例)和卒中组(96例),选取健康志愿者(53例)作为对照组。采用基因测序法测定载脂蛋白E基因第四外显子112位(rs429358)和158位(rs7412)单核苷酸位点多态性。结果 (1)PSD组ε3/ε4基因型频数(24例)高于对照组(7例),ε4/4基因频数(8例)高于卒中组(2例),差异有统计学意义(P<0.05);ε4等位基因频率(24.7%)高于对照组(9.4%)和卒中组(16.1%),差异有统计学意义(P<0.05)。(2)卒中组ε2基因型频率(13.5%)大于PSD组(4.8%),差异有统计学意义(χ2=5.563,P=0.018)。(3)PSD组携带ε4等位基因的HAMD-24评分高于非携带ε4等位基因组,差异有统计学意义(P<0.05)。结论 Apo E基因多态性与PSD存在相关性,ε4等位基因可能是PSD的危险因子,而ε2等位基因是PSD的保护因子。Objective To study the depression and apolipoprotein E gene polymorphism,analyze their correlation in post-stroke depression patients. Methods Patients were divided into post-stroke depression group( 83 cases) and brain stroke group( 96 cases) according to diagnosis standard of national fourth session of cerebrovascular disease academic conference and used the Hamilton's depression scale( HAMD-24),selecting healthy volunteer as control group( 53 cases). Detected the two nonsynonymous single nucleotide polymorphisms( rs429358,rs7412) in Apo E exon 4. Results( 1) The frequency distribution of ε3 /4 genetype and epsilon 4 in post-stroke depression patients was much higher than that in control group( P〈0. 05). The frequency distribution of ε4 /4 genetype in post-stroke depression patients was much higher than that in brain stroke patients( P〈0. 05). The frequency of epsilon 4 in post-stroke depression patients group( 24. 7%) was higher than that in brain stroke group( 16. 1%) and control group( 9. 4%)( all P〈0. 05).( 2) The frequency of epsilon 2 in brain stroke group( 13. 5%) was higher than that in post-stroke depression patients group( 4. 8%)( P〈0. 05).( 3) Compared with ε4 allele carriers and non carriers of PSD,the score of HAMD-24 had significant difference( P〈0. 05). There were no differerce among the ε4 allele carriers and non carriers of PSD in age and so on( P〉0. 05). Conclusion The apolipoprotein E ε4 allele is associated with the depression in post-stroke depression patients. The apolipoprotein E ε4 allele is a risk factor of post-stroke depression,ε2 allele is a protection factor of post-stroke depression.
