人工气胸下电视胸腔镜食管癌切除术253例分析

摘　　要：探讨人工气胸条件下,单腔气管插管行电视胸腔镜(VATS)食管癌切除术的安全性、根治性及可行性。选取2010年10月至2014年12月,我院在人工气胸条件下,单腔气管插管行电视胸腔镜食管癌切除术253例,与同期开胸手术63例进行对照分析。全部病例均顺利完成食管癌切除及纵隔淋巴结清扫,两组病例在手术时间、住院天数、肺部感染率、声嘶发生率方面存在差异(P<0.05);清扫淋巴结个数、乳糜胸发生率、吻合口瘘发生率方面无差异(P>0.05)。结果表明,人工气胸条件下单腔气管插管行食管癌切除,效果更为满意、安全。This paper investigated the safety, radical and feasibility of esophageal carcinoma resection by artificial pneumothorax of single-lumen endotracheal intubation in thoracoscopic. 253 cases of esophageal carcinoma received wedge resection by artificial pneumothorax of single-lumen endotracheal intubation in thoracoscopic. And it was compared with 63 cases of thoracic surgery. The operation were successful for all patients. There were differences in the operative time, length of stay, rate of pulmonary infection, the incidence of hoarseness （P〈0.05） between two groups of patients; there were no differences in the lymph node number, chylothorax incidence, and the incidence of anastomotic leakage（P〉0.05）. Esophageal carcinoma resection by artificial pneumothorax of single-lumen endotracheal intubation in thoracoscopic is more reliable and safer.
